A infecção causada por micobactérias não causadoras de tuberculose (MNT) pode resultar em interações assintomáticas, em alguns casos porém, pode também resultar em infecções sintomáticas, cujas manifestações clínicas são pulmonares, linfáticas, cutaneas ou disseminada. Dentre as espécies que fazem parte do “Complexo” Mycobacterium avium, M. intracellulare é a que está mais relacionada com doenças pulmonares, entretanto, este microrganismo pode acometer diversos órgãos, tanto em pessoas imunocomprometidas como em imunocompetentes. A identificação das espécies do "Complexo" M. avium está se tornando cada vez mais importante, pois faculta a possibilidade do conhecimento de possíveis fontes de infecção ambientais, da virulência desses microrganismos, das diferenças no prognóstico da doença, e inclusive de estratégicas terapêuticas específicas. Além de nos permitir conhecer as espécies mais prevalentes, assim como sua distribuição epidemiológica. Assim sendo, este estudo tem por objetivo, identificar as cepas de M. intracellulare/M. chimaera, através da metodologia do sequenciamento e investigar o perfil epidemiológico dos pacientes. Todas as 154 amostras de 66 pacientes foram sequenciadas utilizando três alvos genéticos (genes 16S rRNA, hsp65 e a região ITS) Dentre os indivíduos de estudo 57 (86,4%) tiveram suas cepas identificadas como M. intracellulare, 5 (7,6%) como M. chimaera, 2 (3,0%) como M. marseillense, 1 (1,5%) como M. timonense e 1 (1,5%) como M. yongonense. Do total de pacientes, 21 (31,8%) foram provenientes da região sul, 16 (24,2%) sudeste, 15 (22,7%) centro-oeste, 13 (19,7%) nordeste, e 1 (1,5%) norte, 45 (68,2%) eram do sexo masculino e 21 (31,8%) do sexo feminino, tendo a idade variando de 26 a 87 anos, média de 62 anos e mediana 61,5 anos. O sexo masculino foi o mais frequente na maioria das regiões, com exceção do nordeste, onde houve predominância do sexo feminino. Existem poucos estudos que investigaram os casos de MNT nacionalmente e ainda mais escassos os que analisaram M. intracellulare/ M. chimaera. Esse fato aumenta a relevância do nosso trabalho, que visa em parte, suprir a carência desse tipo de estudo no Brasil.
Abstract
The infection caused by nontuberculous mycobacteria (NTM) may result in asymptomatic interactions, but in some cases, may also result in symptomatic infections whose clinical manifestations are pulmonary, lymphatic, cutaneous or disseminated. Among the species that are part of the Mycobacterium avium Complex, M. intracellulare is the species that is more related with lung disease, however, this organism can affect different organs in both immunocompromised and immunocompetent individuals. The identification of the species of the M. avium Complex is becoming increasingly important as it provides the possibility of knowledge of possible sources of environmental infection, virulence of these microorganisms, the differences in the prognosis of the disease, and even of specific therapeutic strategies. In addition to allowing us to meet the most prevalent species and its epidemiological distribution. Therefore, this study aims to identify the strains of M. intracellulare/M. chimaera by sequencing methodology and investigate the epidemiological profile of the patients. All 154 samples from 66 patients were sequenced using three genetic targets (16S rRNA, hsp65 and ITS region genes) Among the study subjects 57 (86.4%) had their strains identified as M. intracellulare, 5 (7.6%) as M. chimaera, 2 (3.0%) as M. marseillense, 1 (1.5 %) As M. timonense and 1 (1.5%) as M. yongonense. Of all patients, 21 (31.8%) were from the southern region, 16 (24.2%) southeast, 15 (22.7%) midwest, 13 (19.7%) northeast, and 1 ( 1.5%) north, 45 (68.2%) were men and 21 (31.8%) were women, with ages ranging 26-87 years, mean 62 years and median 61.5 years. Men were most frequent in almost all regions except the northeast, where there was a predominance of women. There are few studies that have investigated the cases of NTM nationally and even scarcer who analyzed M. intracellulare / M. chimaera. This fact increases the relevance of our work, which aims in part, address the lack of such studies in Brazil.
