Abstract in Portuguese | Com um imenso impacto na saúde global, o diagnóstico da malária ainda é um desafio para a maioria dos países onde há incidência da doença. Os Testes de Diagnóstico Rápido (TDRs) de malária, por sua praticidade e facilidade de realização, são extremamente úteis para a confirmação diagnóstica, principalmente em regiões de fronteira internacional. O objetivo deste estudo foi analisar a aplicação dos TDRs de malária no município de Oiapoque, Amapá, entre os anos de 2015 a 2020. A pesquisa compreendeu um estudo descritivo, retrospectivo, com abordagem quali-quantitativa. Foram utilizados dados secundários do Sistema de Vigilância Epidemiológica (SIVEP-Malária) e realizados questionários semiestruturados com informantes-chave. Realizou-se uma análise temática e as categorias analíticas identificadas foram incluídas numa matriz DOFA (Debilidades, Oportunidades, Fortalezas e Ameaças). A configuração da malária se modificou ao longo dos anos do estudo. Os anos de 2017 e 2018 apresentaram maior concentração de casos notificados e autóctones, seguido de uma tendência de redução nos anos posteriores, culminando com o menor número de casos positivos no ano de 2020. Os casos importados da Guiana Francesa, diminuíram consideravelmente, em comparação a outros estudos. O perfil epidemiológico dos pacientes revelou que a malária acometeu, em maior frequência, pessoas do sexo masculino, nas faixas etárias produtivas (de 20 a 29 anos e de 30 a 39 anos), indígenas, com ocupação funcional na agricultura e o Plasmodium vivax como a espécie parasitária predominante. A IPA do município variou entre médio e alto risco de ocorrência anual Verificou-se uma baixa morbidade e zero mortalidade pela doença. A rede de diagnóstico contou com 25 (vinte e cinco) unidades de saúde que realizaram exames de malária (gota espessa e TDRs), distribuídas nas áreas rural, urbana e indígena. Os entraves e desafios na utilização dos TDRs na perspectiva dos informantes-chave foram: a manutenção do acondicionamento correto e do controle real da utilização; a exposição dos testes rápidos a altas temperaturas e a falta de solução tampão, em alguns kits de testes rápidos de um laboratório específico. Como contribuições dos TDRs os atores locais retrataram: a praticidade, facilidade no manuseio; a rapidez e a ampliação do diagnóstico da malária, principalmente em localidades de difícil acesso e sem estrutura laboratorial. É essencial que os setores responsáveis pela vigilância da malária no município trabalhem articulados entre si, com os órgãos de saúde da Guiana Francesa e com outros atores sociais, buscando fortalecer as ações de controle/eliminação da doença no município. | en_US |

Abstract | With a huge impact on global health, diagnosing malaria is still a challenge for most countries where the disease is present. The Rapid Diagnostic Tests (RDTs) of malaria, due to their practicality and ease performance, are extremely useful for diagnose confirmation, especially in international border regions. The objective of this study was to analyze the application of malaria RDTs in the municipality of Oiapoque, Amapá, between 2015 and 2020. The research comprised a descriptive, retrospective study with a qualitative-quantitative approach. Secondary data from the Epidemiological Surveillance System (SIVEP-Malaria) were used and semi-structured questionnaires were conducted with key informants. A thematic analysis was carried out and the analytical categories identified were included in a SWOT matrix (Strengths, Weaknesses, Opportunities, and Threats). The configuration of malaria has changed over the years of the study. The years 2017 and 2018 presented a higher concentration of notified and autochthonous cases, followed by a downward trend in subsequent years, culminating in the lowest number of positive cases in 2020. Cases imported from French Guiana have decreased considerably compared to other studies. The epidemiological profile of the patients revealed that malaria most frequently affected males, in the productive age groups (from 20 to 29 years and from 30 to 39 years), indigenous people, with functional occupation in agriculture and Plasmodium vivax as the predominant parasitic species. The IPA of the municipality varied between medium and high risk of annual occurrence There was a low morbidity and zero mortality from the disease. The diagnostic network had 25 (twenty-five) health units that performed malaria tests (blood drop and RDTs), distributed in rural, urban and indigenous areas. The obstacles and challenges in the use of RDTs from the perspective of the key informants were the maintenance of correct packaging and actual utilisation control; the exposure of rapid tests to high temperatures and the lack of buffer solution in some rapid test kits from a specific laboratory. As contributions of the RDTs the local actors portrayed: the practicality, ease in handling; the speed and expansion of malaria diagnosis, especially in locations of difficult access and without laboratory structure. It is essential that the sectors responsible for malaria surveillance in the municipality work in cooperation with each other, with the health agencies of French Guiana and with other social actors, seeking to strengthen the actions of control/elimination of the disease in the municipality. | en_US |
