INTRODUÇÃO: A imunofenotipagem por citometria de fluxo desempenha um papel importante no diagnóstico, classificação e acompanhamento das Leucemias Mielóides Agudas (LMA). A sua utilização com finalidade prognóstica ainda é algo controverso, apesar de já ter sido identificado perfis imunofenotípicos específicos associados a alterações citogenéticas e moleculares subjacentes. OBJETIVO: Este trabalho teve como objetivo principal avaliar a função prognóstica da citometria de fluxo, através da análise de associação das marcações imunofenotípicas com características clínico-demográficas; laboratoriais; taxas de resposta; sobrevida livre de doença e sobrevida global, de pacientes adultos com LMA. MATERIAIS E MÉTODOS: Foram analisados retrospectivamente 102 casos de LMA diagnosticados, em um dos centros de referência para o tratamento das Leucemias Agudas, no Estado da Bahia, no período de 2015-2021. RESULTADO: Os pacientes submetidos a terapia quimioterápica intensiva, apresentaram taxas de resposta completa (70,6%) e taxa de sobrevida global (SG) em 5 anos (36,5%) semelhantes aos desfechos clínicos reportado na literatura. Foi observado uma frequência elevada de fenótipos aberrantes em torno de 71,6%. Dentre os antígenos anômalos expressos, o mais frequente foi o CD7 (52,6%), seguido pelo o CD19 (12,2%). Avaliando pacientes submetidos à quimioterapia intensiva, a expressão positiva do CD7 foi associada a uma probabilidade aumentada de recidiva e refratariedade, assim como redução significativa da sobrevida livre de doença. Por outro lado, a expressão positiva do MPO foi associada de forma significativa a uma maior taxa de resposta completa e maior sobrevida global A coexpressão dos marcadores CD34pos/CD7pos foi associada com refratariedade/recidiva, enquanto CD7pos/CD56pos foi associada com baixa taxa de resposta completa. Enquanto, a co-expressão positiva dos 4 marcadores pan-mieloides CD13/CD33/CD117/MPO foi associada a uma maior frequência de resposta completa. CONCLUSÕES: Este estudo confirma a associação de alguns importantes marcadores imunofenotípicos com a resposta terapêutica e desfecho de pacientes com LMA, indicando a relevância da utilização da imunofenotipagem na definição de prognóstico. Além disso, foi observado taxas de resposta e sobrevida consistentes com as já reportadas na literatura. Para confirmar alguns achados e avaliar as correlações entre alterações citogenéticas ou moleculares com perfis imunofenotípicos e sua associação prognóstica, será necessário avaliar um número maior de pacientes
Abstract
INTRODUCTION: Flow cytometry immunophenotyping plays an important role in the diagnosis, classification, and follow-up of Acute Myeloid Leukemias (AML). Its use for prognostic purposes is still somewhat controversial, although specific immunophenotypic profiles associated with underlying cytogenetic and molecular alterations have already been identified. OBJECTIVE: The main objective of this study was to evaluate the prognostic function of flow cytometry, by analyzing the association of immunophenotypic markers with clinical-demographic characteristics; laboratory; response rates; disease-free survival and overall survival of adult patients with AML. MATERIALS AND METHODS: We retrospectively analyzed 102 cases of AML diagnosed in one of the reference centers for the treatment of Acute Leukemias, in the State of Bahia, in the period 2015-2021. RESULTS: Patients undergoing intensive chemotherapy had complete response rates (70.6%) and 5-year overall survival (OS) rate (36.5%) similar to clinical outcomes reported in the literature. A high frequency of aberrant phenotypes around 71.6% was observed. Among the anomalous antigens expressed, the most frequent was CD7 (52.6%), followed by CD19 (12.2%). Evaluating patients undergoing intensive chemotherapy, positive expression of CD7 was associated with an increased probability of relapse and refractoriness, as well as a significant reduction in diseasefree survival. On the other hand, positive expression of MPO was significantly associated with a higher complete response rate and higher overall survival Co-expression of CD34pos/CD7pos markers was associated with refractoriness/relapse, while CD7pos/CD56pos was associated with a low rate of complete response. Meanwhile, positive co-expression of the 4 pan-myeloid markers CD13/CD33/CD117/MPO was associated with a higher frequency of complete response. CONCLUSIONS: This study confirms the association of some important immunophenotypic markers with the therapeutic response and outcome of patients with AML, indicating the relevance of the use of immunophenotyping in defining prognosis. In addition, response and survival rates consistent with those already reported in the literature were observed. To confirm some findings and evaluate the correlations between cytogenetic or molecular alterations with immunophenotypic profiles and their prognostic association, it will be necessary to evaluate a larger number of patients
