A hepatite C e a esquistossomose são graves problemas de saúde pública no Brasil.Quando essas doenças estão associadas na comorbidade (CM), a fibrose evoluimais rapidamente para cirrose. As células T regulatórias (T reg) são produzidas empresença de estímulo antigênico e identificadas por marcadores de superfície (CD4+,CD25+) e intracelular (FOXP3+) e pela produção de citocinas IL-10 e TGF-β. Ainda,estão associadas à proteção do tecido hepático contra danos causados pelaresposta imune celular efetora contra o hepatitis C virus (HCV) e Schistosomamansoni. Portanto, o objetivo foi avaliar o perfil de células T reg periféricas naturais(nTreg), induzidas (iTreg) e totais em pacientes com esquistossomose (n=22), comhepatite C (n=25) e com a comorbidade (n=14). O percentual dessas células foiobtido por citometria de fluxo. Níveis de IL-10 e TGF-β foram dosados por ELISA e aexpressão gênica de FOXP3 foi verificada por qPCR (∆Ct comparativo). Houvemaior percentual de nTreg, iTreg e de T reg total no grupo com esquistossomosecomparado ao CM (p = 0,036; 0,037 e 0,023, respectivamente). Também houvediferença entre o percentual de nTreg nos grupos esquistossomose e hepatite C (p =0,006). Ao associar estes resultados ao grau de fibrose nos grupos hepatite C e CM,não houve diferença. Os percentuais de iTreg (p = 0,026) e totais (p = 0,040) forammaiores no grupo esquistossomose em relação ao CM quando associados aopadrão D de fibrose. Nos padrões E/F houve diferença no percentual de nTreg (p =0,046) e T reg totais (p = 0,044), sendo maior também no grupo esquistossomose. AIL-10 manteve-se elevada nos grupos infectados por S. mansoni (p = 0,023). Osgrupos hepatite C (p = 0,001) e CM (p = 0,048) apresentaram níveis elevados deTGF-β em relação ao grupo esquistossomótico. A expressão gênica de FOXP3 foimaior 4,27 e 4,89 vezes nos grupos hepatite C e esquistossomose que na CM.Conclui-se que o grupo com esquistossomose apresentou maior percentual decélulas T reg, produção elevada de IL-10 e baixos níveis de TGF-β em relação aosgrupos hepatite C e CM. Isto sugere que o maior percentual de T reg observado emdoenças com menor morbidade está relacionado a um efeito protetor contra danoscausados ao tecido hepático.
Abstract
Hepatitis C and schistosomiasis are serious public health problems in Brazil. Whenthese diseases are associated in comorbidity (CM), the fibrosis can progress morequickly to cirrhosis. T regulatory cells (T reg) are produced in presence of antigenicstimulation and identified by surface (CD4+, CD25+) and intracellular (FOXP3+)markers and production of cytokines IL-10 and TGF-β. Furthermore, they areassociated with protection against liver tissue damage caused by effectors immuneresponse against HCV and Schistosoma mansoni. The objective was to evaluate theperipheral natural (nTreg), induced (iTreg) and total T reg cells profiles inschistosomiasis (n=22), hepatitis C (n=25) and comorbidity (n=14) patients groups.The proportion of T reg was obtained by flow cytometry. Levels of IL-10 and TGF-βwere obtained by ELISA and gene expression of FOXP3 by qPCR (ΔCtcomparative). There was a higher percentage of nTreg, iTreg and total T reg inschistosomiasis group in comparison to CM group (p = 0.036, 0.037 and 0.023,respectively). Difference between the percentage of nTreg in schistosomiasis andhepatitis C groups (p = 0.006) was observed. When T reg cells were associated withfibrosis degree in hepatitis C and CM, no difference was found. The percentage ofiTreg (p = 0.026) and total T reg (p = 0.040) were higher in schistosomiasis group inrelation to CM group, when associated with D fibrosis pattern. Analyzing E/F fibrosispatterns between these two groups, the percentage of nTreg (p = 0.046) and total Treg (p = 0.044) were higher again in schistosomiasis group. IL-10 remainedincreased in the S. mansoni infected groups (p = 0.023). Hepatitis C (p = 0.001) andCM (p = 0.048) exhibited elevated levels of TGF-β in relation to schistosomiasispatients. The FOXP3 gene expression was 4.27 and 4.89 times greater in hepatitis Cand schistosomiasis than in CM group. It is concluded that schistosomiasis patientshad a higher percentage of T reg cells, high production of IL-10 and low levels ofTGF-β in relation to hepatitis C and CM patients groups. This suggests that thehighest percentage of T reg observed in disease with less morbidity is related to aprotective effect against liver tissue damage.
