A Leishmaniose Visceral (LV) é uma doença infecciosa e sistêmica, considerada um grave problema de saúde pública. No Brasil, diversas epidemias urbanas foram observadas diferentes cidades, sendo precedidas pela ocorrência da Leishmaniose Visceral Canina (LVC) e, associadas a determinantes sociais e ambientais. O estudo tem objetivou analisar a distribuição espacial, temporal e espaço-temporal da LV, nas Regiões Geográficas Imediatas, entre os anos de 2007 até 2020, com base em dados secundários das notificações dos casos da doença no SINAN. Análises considerando a variável tempo foram realizadas através de um modelo de regressão Joinpoint, para investigar tendência e um modelo proposto pelo software EPIPOI, para sazonalidade. Quanto ao espaço, foram feitas análises espaciais exploratórias onde foram apresentadas as taxas de incidência de LV, considerando-se o ponto de corte do Ministério da Saúde, e calculados os Índices Espaciais de Moran Globais e Locais, com as mesmas taxas. E, finalmente, para analisar espaço e tempo conjuntamente, empregou-se a estatística de varredura no espaço e no tempo de Kulldorf e Nagarwalla. As análises temporais apontaram tendência de diminuição da taxa de incidência no Brasil, nas regiões Norte, CentroOeste e Sudeste, manutenção das taxas no Nordeste e aumento no Sul, além disso, a proporção de coinfecção LV/HIV e a letalidade apresentaram tendência de aumento. As análises espaciais e espaço-temporais encontraram cluster espaciais e espaço-temporais nas regiões Nordeste, Centro-Oeste, Norte e Sudeste, identificando-as como importantes áreas de transmissão da doença no Brasil. Portanto, reforçando que a LV não ocorre de forma aleatória no espaço e no tempo.
Abstract
Visceral Leishmaniasis (VL) is an infectious and systemic disease, considered a serious public health problem. In Brazil, several urban epidemics were observed in different cities, preceded by the occurrence of Canine Visceral Leishmaniasis (CVL) and associated with social and environmental determinants. The study aimed to analyze the spatial, temporal and spatiotemporal distribution of VL, in the Immediate Geographic Regions, between the years 2007 to 2020, based on secondary data from notifications of cases of the disease on SINAN. Analyzes considering the time variable were performed using a Joinpoint regression model, to investigate trends, and a model proposed by the EPIPOI software, for seasonality. As for space, exploratory analyzes were carried out in which VL incidence rates were adopted, considering the cutoff point of the Ministry of Health, and Global and Local Moran Spatial Indexes were calculated, with the same rates. And, finally, to analyze space and time together, Kulldorf and Nagarwalla's space and time scan statistics were used. Temporal analyzes showed a tendency for the incidence rate to decrease in Brazil, in the North, Midwest and Southeast regions, maintenance of rates in the Northeast and an increase in the South, in addition, the proportion of VL/HIV coinfection and lethality tending to increase. The spatial and spatiotemporal analyzes found spatial and spatiotemporal clusters in the Northeast, Midwest, North and Southeast regions, identifying them as important areas of disease transmission in Brazil. Therefore, reinforcing that VL does not occur randomly in space and time.
