Abstract in Portuguese | A dengue é uma infecção arboviral causada por um dos quatro sorotipos do vírus dengue (DENV1-4) e resulta em um amplo espectro clínico, que vai desde um quadro febril auto-limitante até as formas mais severas da doença. Os mecanismos imunopatogênicos envolvidos no desenvolvimento das manifestações clínicas da dengue são multifatoriais e não mutuamente exclusivos. Em lactentes, a imunidade inata desempenha um papel central no controle das infecções virais, antes do estabelecimento da imunidade adaptativa. A lectina ligante de manose (MBL), uma proteína essencial da imunidade inata, parece ter um papel crucial nas infecções por DENV. Polimorfismos de base única (SNPs) no gene MBL2, que estão associados a níveis reduzidos da MBL, podem influenciar na susceptibilidade e severidade da dengue. Estudos recentes têm explorado o papel desses SNPs e dos níveis de MBL nas infecções por dengue, mas tais estudos ainda são escassos e controversos, especialmente na população pediátrica. Esse trabalho teve como objetivo avaliar a influência de SNPs no MBL2 e níveis da MBL nas infecções por dengue em lactentes. O estudo incluiu lactentes com dengue sintomática (n=29), assintomática (n=17) e controles naïves (n=84). A dosagem de MBL foi realizada através de ELISA in house e os polimorfismos no gene MBL2 foram determinados através de PCR em tempo real. Os níveis basais e capacidade de ligação da MBL foram menores nos lactentes infectados pelo DENV do que nos controles (p<0,0001). Encontramos associação entre o alelo X (OR: 2,16; 95% IC: 1,12-4,19; p=0,019) e genótipo YX (OR: 2,70; 95% IC: 1,20-5,71; p=0,015) com o risco de dengue nos lactentes. | en_US |

Abstract | Dengue is an arboviral infection caused by one of the four serotypes of the dengue virus (DENV1-4) and results in a broad clinical spectrum, ranging from a self-limiting febrile condition to more severe forms of the disease. The immunopathogenic mechanisms involved in the development of the clinical manifestations of dengue are multifactorial and not mutually exclusive. In infants, innate immunity plays a central role in limiting viral infections before the establishment of adaptive immunity. Mannose-binding lectin (MBL), an essential protein of the innate immune system, appears to play a crucial role in DENV infections. Single-nucleotide polymorphisms (SNPs) in the MBL2 gene, which are associated with reduced serum levels of MBL, can influence dengue susceptibility and severity. Recent studies have explored the role of these SNPs and MBL levels in dengue infections, but such studies are still scarce and controversial, especially in the pediatric population. This work aimed to evaluate the influence of MBL SNPs and serum levels on dengue infections in infants. The study included infants with symptomatic (n=29) and asymptomatic (n=17) dengue infection and naïve-control infants (n=84). Measurement of MBL levels was carried out by ELISA in house assays and the polymorphisms in the MBL2 gene were determined through real-time PCR. MBL basal levels and binding capacity were lower in infants infected with DENV than in controls (p<0.0001). We found an association between the X allele (OR: 2.16; 95% CI: 1,12-4,19; p=0.019) and YX genotype (OR: 2.70; 95% CI: 1,20-5,71; p=0.015) with risk of dengue in infants. In contrast, the O allele (OR: 0.41; 95% CI: 0,21-0,77; p=0.006) and the AO genotype (OR: 0.45; 95% IC: 0,23-0,91; p=0.021) were more frequent in the control group and associated with protection. Together, these data suggest that high to intermediate levels of MBL seems to be protective against DENV infection in infants up to two years of age. | en_US |
