INTRODUÇÃO: Os sinais e sintomas de uma patologia e as reações adversas de seu tratamento podem causar estresse, baixa da autoestima, mau-humor, desânimo, entre outras sensações, com potencial de afetar direta ou indiretamente a qualidade de vida. Por acometer o aparelho reprodutor, o diagnóstico de Câncer do Colo do Útero (CCU) pode impactar sobre a feminilidade e sexualidade, comprometendo ainda mais o estado emocional da mulher, com possíveis reflexos em sua condição física, psicológica e relações sociais. OBJETIVOS: Descrever aspectos psicoemocionais e a qualidade de vida de mulheres com câncer do colo do útero assistidas em um hospital de referência de Salvador bem como caracterizar o perfil epidemiológico, sociodemográfico e clínico. MÉTODOS: Trata-se de um estudo transversal, realizado em um grupo de mulheres com idade ≥18 anos e com diagnóstico confirmado de CCU após o primeiro atendimento no ambulatório de oncoginecologia de um hospital de referência. A inclusão de participantes foi realizada entre julho de 2018 e outubro de 2019. Por meio de entrevista estruturada e revisão de prontuário, foram coletados dados sociodemográficos, histórico de tabagismo, vida sexual, antecedentes ginecológicos e sinais e sintomas relacionados à doença. Entre janeiro de 2019 e março de 2020, seis meses após a data de inclusão das participantes no estudo, foram obtidos dados psicoemocionais e de qualidade de vida por meio de ligações telefônicas e uso do questionário World Health Organization Quality of Life-Bref (WHOQOL-Bref). RESULTADOS: Foram identificadas 179 pacientes com diagnóstico confirmado de CCU, sendo que para 159 delas foi possível obter dados para contato telefônico. Foi possível completar o protocolo para contato telefônico (pelo menos cinco tentativas de ligação, realizadas em dias e horários alternados) para 115 das 159 pacientes, sendo que os contatos foram finalizados com êxito na obtenção de dados psicoemocionais e de qualidade de vida para 74 (64,3%) delas; para 41 (35,7%) não foi possível realizar a entrevista. Foi observado que a maioria das mulheres que responderam a entrevista sobre a qualidade de vida e aspectos psicoemocionais consideravam-se espiritualizada ou religiosa (94,4%) e a religião/crença mais referida foi a católica (57,8%), seguindo da evangélica (29,6%). A espiritualidade foi considerada pela grande maioria como apoio no enfretamento da doença. Alteração no humor (75,7%), insegurança (71,6%), choro fácil (69,0%), alteração no padrão alimentar (62,2%), alteração no sono (60,8%), medo da morte (59,5%) foram sentimentos ou sensações negativas frequentemente relatados após o diagnóstico do CCU. A qualidade de vida global das mulheres apresentou um escore (61,2) semelhante aos domínios físico (62,4), psicológico (63) e relações sociais (68). No entanto, o domínio relacionado ao meio ambiente, apresentou o menor escore (55,3), entre todos os analisados. CONCLUSÃO: Pode-se observar que incertezas e medo estão presentes após o diagnóstico, desencadeando um processo de dúvidas e angústias. A religiosidade foi uma das estratégias utilizadas por essas mulheres para o enfrentamento desse momento complexo e doloroso fisicamente e emocionalmente. A pior avaliação do domínio do ambiente sugere que as características do local de residência afetam negativamente a qualidade de vida destas mulheres.
Abstract
INTRODUCTION: The signs and symptoms of a pathology and the adverse reactions of its treatment can cause stress, low self-esteem, bad mood, discouragement, among other sensations, with the potential to directly or indirectly affect the quality of life. As it affects the reproductive system, the diagnosis of Cervical Cancer (CC) can impact on femininity and sexuality, further compromising the woman's emotional state, with possible reflexes on her physical, psychological and social relationships. OBJECTIVES: To characterize the epidemiological, sociodemographic and clinical profile of women with UCC assisted at a referral hospital in Salvador and to describe psycho-emotional aspects related to their quality of life after diagnosis. METHODS: This is a cross-sectional study, carried out in a group of women aged ≥18 years and with a confirmed diagnosis of UCC after the first visit to the oncogynecology outpatient clinic of a reference hospital. The inclusion of participants was carried out between July 2018 and October 2019. Through structured interviews and review of medical records, sociodemographic data, smoking history, sexual life, gynecological history and signs and symptoms related to the disease were collected. Between January 2019 and March 2020, six months after the date of inclusion of the participants in the study, psycho-emotional and quality of life data were obtained through telephone calls and use of the World Health Organization Quality of Life-Bref (WHOQOL-Bref). RESULTS: 179 patients with a confirmed diagnosis of CC were identified, and 159 of them were able to obtain data for telephone contact. It was possible to complete the protocol for telephone contact (at least five connection attempts, performed on alternate days and times) for 115 of the 159 patients, and the contacts were successfully completed in obtaining psycho-emotional and quality of life data for 74 (64.3%) of them; for 41 (35.7%) it was not possible to conduct the interview. It was observed that the majority of women who answered the interview about quality of life and psycho-emotional aspects considered themselves to be spiritual or religious (94.4%) and the most mentioned religion / belief was Catholic (57.8%), followed by evangelical (29.6%). Spirituality was considered by the vast majority as support in coping with the disease. Change in mood (75.7%), insecurity (71.6%), easy crying (69.0%), change in eating pattern (62.2%), change in sleep (60.8%), fear of death (59.5%) were negative feelings or sensations frequently reported after the diagnosis of CC. The overall quality of life of women presented a score (61,2) similar to the physical (62,4), psychological (63) and social relationships (68) domains. However, the domain related to the environment, presented the lowest score (55.3), among all analyzed. CONCLUSION: It can be observed that uncertainties and fear are present after the diagnosis, triggering a process of doubts and anxieties. Religiosity was one of the strategies used by these women to face this complex and painful moment, physically and emotionally. The worst assessment of the domain of the environment suggests that the characteristics of the place of residence negatively affect the quality of life of these women.
