A dissertação trata das transformações e limites nos modos de lidar com as situações
de crise na Reforma Psiquiátrica, a partir de dois serviços de atenção psicossocial: um
Hospital-dia e um Centro de Atenção Psicossocial. O trabalho problematiza a própria noção
de crise como expressão utilizada para nomear experiências singulares diversas e que tem
implicações nos modos de viver essas experiências. As situações de crise/surto foram
tomadas como analisadoras das relações com a loucura e das “novas” práticas em saúde
mental e atenção psicossocial por constituírem o principal motivo pelo qual as pessoas
ingressam no circuito psiquiátrico ou na rede de saúde mental e um desafio para os
profissionais, sendo apontado na literatura como momento de máxima simplificação de
questões sociais, culturais e existenciais das pessoas atendidas numa situação configurada
como de emergência psiquiátrica.
A formulação das questões da pesquisa, as análises das entrevistas e a observação
participante no cotidiano dos serviços basearam-se no Referencial Teórico da Genealogia
de Michel Foucault, nos teóricos da Reforma Psiquiátrica Italiana, nas análises de
Amarante (1995, 1996) sobre a Reforma Psiquiátrica Brasileira, e outros autores, tais como
Castel (1997) e Certeau (1994).
Quanto às dificuldades para lidar com as situações de crise, desconstruindo e
problematizando a necessidade do manicômio e da internação, os profissionais sentem mais
limites em algumas situações específicas: quando o usuário está recentemente vinculado ao
serviço, quando ele e/ou sua família ainda possuem forte referência no hospital como
recurso ou quando freqüenta pouco o serviço. Foi possível analisar como a concepção dos
profissionais sobre o serviço tem implicações nos modos de lidar com as situações de crise.
Existe menor transformação no lidar com as experiências de maior intensidade expressa,
cujo destino para a internação não é raro. O fato dos serviços estarem ainda mais voltados
para o seu interior, com uma rede precária e um trabalho territorial reduzido atravessa
significativamente as situações de crise e coloca limites às transformações nesse sentido.
Por fim, nota-se a necessidade permanente de reflexão ética e política sobre o lugar social
da loucura e o modo como as práticas de atenção psicossocial vêm se construindo.
Abstract
This dissertation discourses on the transformations and limits on the ways of dealing with
crisis situations in the Psychiatric Reform, based on two psychosocial atention services: a
Day-hospital and a Psychosocial Atention Centre. This piece of work problematizes the
very notion of crisis as an expression used to name diverse singular experiences and that
has implications on the ways of living these experiences. The outbreak/crisis were taken as
analysers of the relations with madness and the “new” practices in mental health and
psychosocial atention because they constitute the main reason why people enter in the
psychiatric circuit or in the mental health network and are a challenge for the professionals,
being appointed in the literature as a moment of maximum simplification of social, cultural
and existential issues of the attended people in a situation configured as a psychiatric
emergency.
The formulation of the research issues, the analysis of the interviews and the participating
observation in the day-to-day of the services were based on the Genealogical Theoretical
Reference of Michel Foucault, on the Italian Psychiatric Reform theorists, on the analysis
of Amarante (1995,1996) over the Brazilian Psychiatric Reform, and other authors such as
Castel (1997) and Certeau (1994).
As to the difficulties to deal with crisis situations, deconstructing and problematizing the
necessities of the asylum and internation, the professionals feel more limits in some specific
situations: when the user has been recently linked to the service, when he/she or his/her
family still have a strong reference on the hospital as a resource or when he/she don’t go
frequently to the service. It was possible to analyse how a conception of the professionals
over the service has implications on the ways of dealing with crisis situations. There is less
change in the dealing with experiences of more expressed intensity, whose destiny to
internation isn’t rare. The fact that the services are even more turned to their interior, with a
precarious net and a territorial work reduced crosses significantly the crisis situations and
sets limits to the transformation in this sense.
Finally, it can be noted that there is a permanent necessity of ethical and political reflection
on the social place of madness and on the way the psychosocial atention practices are
being constructed.
