Abstract in Portuguese | A interação entre a tuberculose (TB) e o vírus da imunodeficiência humana (HIV) tem sido um grande desafio para a saúde pública, considerando sua magnitude e os altos índices de mortalidade por tuberculose entre pessoas que vivem com HIV (PVHIV). A Organização Mundial de Saúde (OMS) tem recomendado a triagem de TB em PVHIV a partir do questionamento de quatro sintomas relacionados à TB (tosse, febre, sudorese noturna e perda de peso). Com o objetivo de analisar a efetividade dessa triagem foi realizado ensaio clínico pragmático com randomização individual, no Hospital Correia Picanço, hospital de referência para HIV no Estado de Pernambuco. Sendo selecionados pacientes recém diagnosticados para HIV, com idade \2265 18 anos, residentes de Pernambuco, acompanhados neste serviço, com menos de um mês de uso de terapia antirretroviral e sem TB ativa. Foram excluídos os pacientes privados de liberdade e aqueles com teste confirmatório para o HIV negativo. O período de estudo foi de março/2014 a abril/2016, sendo acompanhados por pelo menos seis meses. Os resultados demonstram que a triagem teve moderada sensibilidade (85,3%), baixa especificidade (46,4%) e baixo valor preditivo positivo (13,6%), porém alto valor preditivo negativo (97%). As taxas de detecção de TB foram similares na intervenção e no controle: 9% (IC 95%: 6,3%-12,4) e 9,3% (IC 95%: 5,7%-14,2%) respectivamente (p=0,906). Os tempos entre o diagnóstico do HIV e o início do tratamento para TB foi menor na intervenção (40 dias contra 50 dias do grupo controle), porém sem diferença estatística. A sobrevida global nos primeiros seis meses de acompanhamento após o diagnóstico do HIV foi: 94% até o final do seguimento, com 93,1% para intervenção e 95,1% para o controle (p=0,343). A triagem recomendada pela OMS se mostrou pouco efetiva a detecção de TB e para os desfechos de tratamento no cenário estudado. Não demonstrando impacto efetivo na sobrevida global durante os primeiros seis meses de acompanhamento (AU). | pt_BR |

Abstract | The interaction between tuberculosis (TB) and human immunodeficiency virus (HIV) has been a major public health challenge, considering its magnitude and high tuberculosis mortality rates among people living with HIV (PLHIV). The World Health Organization (WHO) has recommended the screening of TB in PLHIV by questioning signs and symptoms related to TB (cough, fever, night sweats and weight loss). With the objective of analyzing the effectiveness of this screening, a pragmatic clinical trial with individual randomization was performed at the Correia Picanço Hospital, a reference hospital for HIV in the State of Pernambuco. Patients newly diagnosed for HIV, aged \226518 years, who were residents of Pernambuco, were enrolled in this service, with less than one month of antiretroviral therapy and no active TB. Patients deprived of liberty and those with a confirmatory HIV-negative test were excluded. The study period was from March / 2014 to April / 2016, being followed up for at least six months. The results showed that the screening had moderate sensitivity (85.3%), low specificity (46.4%) and low PPV (13.6%), but high NPV (97%). Detection rate of TB were similar in both groups: 9% (95% CI: 6.3% -12.4) and 9.3% (95% CI: 5.7% -14.2%) respectively (p = 0.906). The time between HIV diagnosis and initiation of treatment for TB was lower in the intervention (40 days versus 50 days in the control group), but with no statistical difference. Survival in the first six months of follow-up after HIV diagnosis was 94% by the end of follow-up, with 93.1% for intervention and 95.1% for control (p = 0.333). The WHO-recommended screening was not effective for the detection of TB, nor for the treatment outcomes in the scenario studied.
