O diabetes é uma doença crônica que acarreta altas taxas de internação. O controle ina-dequado da glicemia durante o intra-hospitalar está associado a maior tempo de internação e desfechos desfavoráveis, culminando com o aumento do custo. O objetivo deste trabalho foi realizar um estudo de avaliabilidade para uma avaliação de custo-efetividade de um protocolo de hiperglicemia em pacientes internados em um hospital público do Distrito Federal em situação de pandemia por COVID-19. O desenho desta avaliação seguiu o recomendado para um estudo de avaliabilidade, com a fragmentação e explanação da elaboração do modelo lógico e descritivo da intervenção, caracterização dos stakeholders, definição das perguntas avaliativas e elaboração do modelo da avaliação. Para a realização do estudo, foram consultados documen-tos institucionais, realizado revisão bibliográfica relacionado ao tema e análise de prontuários, achados de um piloto em condições reais de infra estrutura e interacionais de uma unidade hospitalar do DF. Os principais achados foram a construção do modelo lógico da intervenção, um protocolo de hiperglicemia para pacientes internados, o modelo de avaliação de um estudo de custo-efetividade desta intervenção considerando o contexto de um hospital público no DF e as lições aprendidas para o planejamento e a implementação de avaliações em situação de crise sanitária. O modelo lógico da intervenção resultou da revisão bibliográfica, principalmente o posicionamento oficial da SBD de 2015 “Controle da Glicemia no Paciente Hospitalizado” associado aos achados do estudo piloto e a validação com os stakeholders; o modelo de avaliação de custo-efetividade estruturado utilizando a revisão bibliográfica, o modelo lógico da intervenção e os achados do estudo piloto. A principal lição aprendida foi identificar a necessidade de aproximação, interação e ação colaborativa com os diferentes stakeholders, tanto na formulação das perguntas avaliativas, na modelização da intervenção quanto no processo de avalição.
Abstract
Diabetes is a chronic disease that leads to high rates of hospitalization. Inadequate gly-cemia control during in-hospital care is associated with longer hospital stays and unfavorable outcomes, culminating in increased cost. The aim of this study was to carry out an evaluability study for a cost-effectiveness assessment of a hyperglycemia protocol in patients admitted to a public hospital in the Federal District in a situation of pandemic by COVID-19. The design of this assessment followed what was recommended for an avaliability study, with the fragmenta-tion and explanation of the elaboration of the logical and descriptive model of the intervention, characterization of the stakeholders, definition of evaluative questions and elaboration of the evaluation model. To does the study, institutional documents were consulted, a bibliographic review related to the topic and analysis of medical records were carried out, findings of a pilot in real infrastructure and interactional conditions of a hospital unit in the Federal District. The main findings were the construction of the logical model of the intervention, a hyperglycemia protocol for hospitalized patients, the evaluation model of a cost-effectiveness study of this intervention considering the context of a public hospital in the DF and the lessons learned for planning and the implementation of assessments in situations of health crisis. The logical model of the intervention resulted from the literature review, mainly the official position of the 2015 SBD “Control of Glycemia in Hospitalized Patients” associated with the findings of the pilot study and validation with stakeholders; the structured cost-effectiveness evaluation model using the literature review, the logical intervention model and the findings of the pilot study. The main lesson learned was to identify the need for approximation, interaction and collaborative action with the different stakeholders, both in the formulation of evaluative questions, in the interven-tion modeling and in the evaluation process.
