A malária tem sido um dos mais importantes problemas para a saúde pública no
Brasil, tendo na década de 90 registrados em média 500 mil casos ao ano, com 99,7% deles
ocorrendo na Amazônia Legal. Em 1999 a situação se agravou e o número de casos atingiu
637 mil. Diante deste quadro o Ministério da Saúde lançou, em julho de 2000, o Plano de
Intensificação das Ações de Controle da Malária (PIACM).
O objetivo deste trabalho é avaliar os resultados obtidos com a implantação do
PIACM, no contexto da descentralização das ações de vigilância epidemiológica e controle
de doenças, nos municípios e estados da Região Amazônica no período de julho de 2000 a
dezembro de 2002. Foram testadas três hipóteses: (i) O Plano de Intensificação das Ações
de Controle da Malária foi efetivo na redução de casos e da Incidência Parasitária Anual de
malária na Região e nos estados da Amazônia Legal (ii) O PIACM foi efetivo na redução
do número de municípios de alto risco de transmissão na Região da Amazônia Legal (iii) O
PIACM foi mais efetivo nos municípios da Amazônia Legal certificados para executar
ações de vigilância em saúde do que nos não certificados, segundo tamanho da população.
Trata-se de um estudo descritivo ecológico que analisou o contexto em que ocorreu
a expansão da infra-estrutura e mudanças nos processos dos serviços de saúde nos estados,
bem como o impacto alcançado na redução da incidência da malária nos estados e
municípios beneficiados pelo plano. Priorizou-se aprofundar o estudo nos municípios
responsáveis por 80% dos casos de malária na Região da Amazônia Legal, estratificando-os
segundo tamanho da população e condição de certificação.
Constatou-se a efetividade do plano na redução da morbi-mortalidade por malária
nos estados da Região Amazônica por intermédio da diminuição do número de casos e de
municípios considerados de alto risco para transmissão da malária. Os municípios pequenos
e médios apresentaram melhor desempenho nos indicadores malariométricos, que os
maiores. Verificou-se também que os resultados nos municípios de médio e pequeno porte
não certificados foram melhores que nos certificados, ao contrário nos de grande porte.
Concluiu-se que no período do PIACM alterou-se a dinâmica da transmissão da malária na Região Amazônica, promovendo diminuição da dispersão da doença na região, tornando-a
mais vulnerável às ações de controle.
Abstract
Malaria has been one of the most important public health problems in
Brazil, with an average of 500 thousand cases registered in the 1990s, with 99.7% of them
click on the Legal Amazon. In 1999 the situation worsened and the number of family cases
637 thousand. This framework of the Ministry of Health launched, in July 2000, the Plan of
Intensification of Malaria Control Actions (PIACM).
The objective of this work is to evaluate the results obtained with the implementation of the
PIACM, in the context of the decentralization of epidemiological surveillance and control actions
of diseases, in the municipalities and states of the Amazon region in the period from July 2000 to
December 2002. Three hypotheses were tested: (The Action Intensification Plan)
of Malaria Control was effective in reducing cases and the Annual Parasitic Incidence of
malaria in the Region and in the states of the Legal Amazon (ii) PIACM was effective in reducing
of the number of municipalities at high risk of transmission in the Legal Amazon Region (iii) The
PIACM was more effective in the municipalities of the Legal Amazon certified to carry out
health surveillance actions than in non-certified ones, according to population size.
It is a descriptive study that is considered a descriptive context
