A criança encontra-se em crescimento e desenvolvimento físico, cognitivo e emocional, sendo este reflexo de um bom estado de saúde em seu conceito mais amplo. Elas são intrinsecamente vulneráveis uma vez que o seu desenvolvimento até a fase adulta sofre o impacto do meio em que vive, das alterações biológicas, relações interpessoais e da sociedade. O processo saúde-doença ganha relevo dentro deste contexto, bem como uma assistência em saúde efetiva que atenda suas necessidades. A adesão a farmacoterapia é fundamental para o sucesso do tratamento e envolve formas farmacêuticas que favoreçam a aceitabilidade pelo paciente pediátrico. É comum na clínica a utilização de medicamentos licenciados para o uso adulto em crianças, tornando a prescrição insegura, e assim elevando a possibilidade de reações adversas graves ou fatais. As entidades sanitárias têm grande dificuldade quanto à regulação de medicamentos de uso pediátrico devido a limitações éticas que norteiam os ensaios clínicos, em especial o de bioequivalência. Além disto, questões como flexibilidade de dose, via de administração e palatabilidade tornam-se desafios acerca do desenvolvimento de formulações para este público. No Brasil não há regulamentação especifica sobre a temática abordada, entretanto o FDA (Food and Drug Administration) e o EMA (European Medicines Agency) possuem instrumentos legais que fomentam a pesquisa e desenvolvimento de formas farmacêuticas voltadas para crianças. Elas analisam a possibilidade da realização de ensaios clínicos pediátricos e, uma vez que a empresa dá seguimento ao estudo, e seus resultados se apresentarem favoráveis, concede-se o licenciamento com extensão da exclusidade de mercado. As informações obtidas na pesquisa clínica são fundamentais para salvaguardar a ética, segurança e eficácia das formulações infantis. Além disso, a disponibilidade destas formulações extende seus benefícios aos pacientes que possuem dificuldade de deglutição, como idosos e a portadores de transtornos mentais, por exemplo, agregando uma vantagem, do ponto de vista econômico, para as empresas fabricantes.
Abstract
The child is in growth and physical development, cognitive and emotional, being this reflection of a good state of health in its broader concept. Tover are intrinsically vulnerable since their development into adulthood is impacted by the environment in which they live, biological changes, interpersonal relationships, and society. The health-disease process gains prominence within this context, as well as effective health care that meets their needs. Adherence to pharmacotherapy is fundamental to the success of the treatment and involves pharmaceutical forms that favor the acceptability by the pediatric patient. It is common in the clinic to use licensed medications for adult use in children, making prescribing unsafe, and thus raising the possibility of serious or fatal adverse reactions. Health authorities have great difficulty in regulating pediatric medicinal products because of the ethical limitations that guide clinical trials, especially bioequivalence. In addition, issues such as dose flexibility, route of administration and palatability become challenges regarding the development of formulations for this audience. In Brazil, there is no specific regulation on the subject matter, but the Food and Drug Administration (FDA) and the European Medicines Agency (EMA) have legal instruments that promote the research and development of pharmaceutical forms for children. They analyze the possibility of conducting pediatric clinical trials, and once the company follows up the study, and its results are favorable, it is granted the license with extension of market exclusivity. The information obtained in clinical research is fundamental to safeguarding the ethics, safety and efficacy of infant formulations. In addition, the availability of these formulations extends their benefits to patients who have difficulty swallowing, such as the elderly and those with mental disorders, for example, by adding an economic advantage to manufacturing companies.
