A detecção oportuna de doenças em viajantes internacionais é primordial para que novas
patologias sejam identificadas antes de se tornarem um risco em potencial à população. A
iminência dos eventos de massa no país e o consequente aumento do fluxo de viajantes
internacionais acarretarão um provável aumento dos riscos de saúde pública relacionados à
disseminação de doenças. Para isso, torna-se fundamental a estruturação dos serviços de
saúde, em especial daqueles que realizarão atendimentos aos turistas, para que estejam aptos
a identificar eventos de interesse à saúde pública e a manejá-los de forma segura e oportuna,
a fim de evitar disseminação e/ou introdução de doenças no país. O estudo foi realizado em
três hospitais da rede pública de saúde da cidade de Brasília, por ser uma das cidades-sedes
da Copa do Mundo de 2014 e também da Copa das Confederações, em 2013, concentrando
grande número de jogos desses eventos. Foram realizadas entrevistas com profissionais que
atuam na emergência, responsáveis técnicos de laboratório, com os coordenadores dos
Núcleos de Vigilância Hospitalar e das Comissões de Controle de Infecção Hospitalar. Foram
coletados dados sobre a estrutura dos hospitais e sobre o fluxo e organização da vigilância de
eventos, doenças e agravos de notificação para diagnóstico da capacidade instalada dos
hospitais e do sistema de vigilância. Observou-se que os hospitais necessitam de adequações
na sua infraestrutura para realizar as ações de vigilância oportunamente e para desenvolver
medidas de controle eficientes. O principal problema diz respeito aos fluxos de notificação,
que necessitam ser revistos e divulgados aos profissionais de saúde. Dentre os 27
profissionais (40,3%) que afirmaram conhecer os fluxos, oito (29,6%) não o descreveram, quando solicitado. Dos 19 profissionais que desenharam o fluxo, apenas 2 (10,5%) o
descreveram de forma correta do ponto de vista das instâncias envolvidas. Com relação às
notificações realiadas no período de estudo, foi observado que a maior parte das notificações
realizadas no Distrito Federal referia- se a eventos de relevância local ou que não constam
dos anexos I e II da Portaria MS n° 104/11. Nas regionais de saúde sul e norte, onde se
localizam os hospitais do estudo, destacam-se as proporções elevadas de notificações
imediatas, enquanto nos hospitais do estudo, sobressaem-se as proporções de doenças de
notificação compulsória não imediatas. Dos eventos notificados, 53,8% dos eventos não
foram notificados ao SINAN. Após análise, foi verificado que dos 2570 eventos notificados
ao SINAN, menos de 1% (0,4) foi notificado ao CIEVS através do SIME.
Abstract
The timely detection of diseases in international travelers is paramount that new diseases
are identified before they become a potential risk to the population. The imminence of mass
gatherings in the country and the consequent increase in the flow of international travelers
likely will lead to increased public health risks related the spread of diseases. For this, it is
crucial to organize health services, especially those who attend tourists, to those services
are able to identify events of interest to public health and to manage them safely and timely
in order to avoid spreading and / or introduction of disease in the country. The study was
conducted in three hospitals of the public health system of the city of Brasilia to be one of
the host cities of the 2014 World Cup and also the Confederations Cup in 2013,
concentrating a large number of games these events. Interviews were conducted with
professionals working in emergency, responsible laboratory technicians, coordinators of the
Epidemiological Surveillance Hospital Core and Monitoring and Control Committee
Hospital Infection Data were collected on the structure of hospitals and on the flow and
organization of monitoring events, diseases and illnesses notification for diagnostic
capacity of hospitals and surveillance system. It was observed that hospitals need to be
adjusted in its infrastructure to conduct surveillance actions in due course and to develop
control measures efficiently. The main problem concerns the notification flows that need to
be reviewed and shared health professionals. Among 27 professionals (40,3%) who
claimed to know the flow, 8 professionals (29,6%) not described the flow; 19 professionals
who designed the flow, only 2 (10,5%) described it correctly from the point of view of the
actors involved. With respect to notifications in the period of study, it was observed that
most of the notifications made in the Federal District were related to events of local relevance or not listed in annexes I and II of the Portaria MS n° 104/11. In the southern
and northern regional health, the locations of hospitals in the study, we highlight the high
proportions of immediate notification, while among the hospitals in the study, the largest
number of proportions were related to not immediate reportable diseases. Among the events
notified, 53.80% of the events were reported to SINAN. After analysis, it was found that of
the 2570 events reported to SINAN, less than 1% (0.42) was reported to CIEVS by SIME.
