PROPOSTAS DE METODOLOGIA ALTERNATIVA IN VITRO PARA ENSAIO DE POTÊNCIA DO MEDICAMENTO CONTENDO FILGRASTIM

O Filgrastim é um biofármaco que compõe um biomedicamento constante na lista de excepcionais adquiridos pelo SUS e está entre os imunobiológicos importados de alto custo. Este biofármaco é uma proteína recombinante humana altamente purificada, baseada no fator estimulador de colônias de granulócitos humano (G-CSF), que tem como ação principal estimular a proliferação e ativação de granulócitos, através de um receptor específico, conhecido como G-CSFR (receptor do fator estimulador de colônias de granulócitos). Atualmente, este biomedicamento não possui monografia na Farmacopéia Brasileira e não é realizado o controle de qualidade biológico deste bioproduto no Brasil. Desta forma, os objetivos deste trabalho foram estabelecer o método para controle de qualidade biológico do Filgrastim preconizado pela Farmacopeia Europeia (FE) no INCQS, desenvolver métodos alternativos ao descrito nesta farmacopéia através da utilização de citometria de fluxo por ensaios de proliferação, ciclo celular e marcação intracelular e testar uma linhagem humana (Kasumi-1) como alternativa a linhagem murina (M-NFS-60) sugerida pela FE. Os resultados dos ensaios de MTT com ambas as linhagens satisfazeram os critérios de regressão como linearidade e coeficientes de determinação (R²). Quando comparados entre os ensaios através da inclinação da reta, ambas as linhagens se encontram dentro dos limites, mostrando que para ambas as linhagens os ensaios com MTT apresentaram precisão intra e inter ensaio, sendo assim possível o seu estabelecimento no INCQS. Quando realizados ensaios de proliferação e ciclo celular com CFSE e com iodeto de propídio (PI) por citometria de fluxo, não foram observadas diferenças significativas de proliferação entre as células sem estímulo e tratadas com Filgrastim para a linhagem M-NFS-60, demonstrando que talvez estes ensaios não sejam adequados para ensaios de potência com esta linhagem. Já a linhagem Kasumi-1 no tempo de 48h de estímulo apresentou diferença entre as diferentes concentrações de Filgrastim, apresentando uma regressão significativa e determinação de 0,9 no ensaio de CFSE e apenas uma tendência no ensaio com PI. Para ambas as linhagens nos ensaios de ativação dos fatores de transcrição do G-CSFR a quantificação de STAT3 e STAT5 com diferentes concentrações de Filgrastim apresentou uma boa correlação (R²>0,7) e com uma relação concentração-resposta visível. Por fim, os ensaios de proliferação por CFSE e ciclo celular por PI foram padronizados, porém os mesmos não foram capazes de apresentar uma dose-resposta em relação a proliferação das células, sendo considerados não adequados para ensaio de potência do biofármaco Filgrastim. Os ensaios de ativação dos fatores de transcrição apresentaram tendências promissoras a uma concentração-resposta, confirmando a hipótese de que o ensaio de MTT visualiza para este biofármaco, mais a ativação celular do que a proliferação. Por fim, foi possível atingir todos os objetivos deste trabalho, como desenvolvimento, estabelecimento e padronização dos ensaios, ficando em pendência a validação destes ensaios com o padrão de referência. Os ensaios por citometria de fluxo com a marcação dos fatores de sinalização STAT3 e STAT5 se mostraram promissores para verificação de potência do biofármaco Filgrastim, assim como a linhagem Kasumi-1.
Abstract
Filgrastim is a biopharmaceutical which takes part in the list of special medications acquired by SUS and is among the imported immunobiological lowest cost. This biopharmaceutical is formed by a recombining human protein highly purified, based on the granulocyte-colony stimulating factor (G-CSF), which has the main function of stimulate the granulocyte proliferation and activation, through a specific receptor, known as G-CSFR (granulocyte-colony stimulating factor receptor). Currently, this biopharmaceutical doesn’t have a monograph in the Brazilian Pharmacopoeia and the biological quality control is not realized in Brazil. The goals of this work were: to establish the method to biological quality control of Filgrastim from European Pharmacopoeia at INCQS; to develop alternative methods from the descripted in this Pharmacopoeia through the usage of flow cytometry by proliferation assays, cell cycle and intracellular staining and to test a human lineage (Kasumi-1) as an alternative to the murine lineage (M-NFS-60) suggested by European Pharmacopoeia (FE). The results of MTT assays with both the lineages presented good linearity e coefficients of correlation. When compared among the assays through slope of the line, both lineages were between the limits, showing that for both cells lines the MTT assay presented intra and inter-assay precision, thus being possible to establish it in the INCQS. When proliferation and cell cycle assays were performed with CFSE and propidium iodide (PI) by flow cytometry, no significant differences in proliferation were observed between cells without stimulation and Filgrastim treated for the M-NFS-60 lineage, demonstrating that maybe these assays not be the most suitable for potency assays with the cell line suggested by FE. However, the lineage Kasumi-1, at the 48h time of stimulus, presented difference between the different concentrations, presenting a significant regression and correlation of 0,9 in the CFSE assay and only a tendency in the PI assay. For both lineages, in the assays of activation of the transcription factors from G-CSFR the quantification of STAT3 and STAT5 with different doses of Filgrastim presented a good correlation (R2>0,7) and with a visible relation concentration-answer. Finally, the assays of proliferation by CFSE and cell cycle by PI were standardized, but the same were not able of presenting a concentration-answer in relation to the cells’ proliferation, being considered not suitable for potency testing of the Filgrastim. The activation assays of factors of transcription has shown themselves with strong tendencies to a concentration-answer, confirming the hypothesis that the MTT assay sees for this biopharmaceutical, more the cell activation than the proliferation. Lastly, was possible achieve all the goals of this work, such as development, establishment and standardization of assays, remaining only the validation of these assays with the reference pattern. The assays by flow cytometry with the staining of signaling factors STAT3 and STAT5 have shown themselves promising to the potency verification of the biopharmaceutical Filgrastim, just as the lineage Kasumi-1.
