Introdução: A prevalência da obesidade infantil vem aumentando nas últimas décadas,
configurando-se como uma preocupação mundial. Para enfrentá-la, têm sido
promovidas estratégias de estímulo à alimentação saudável, além do interesse por
alimentos funcionais, como a aveia.
Objetivo: Analisar a mudança nos perfis antropométrico e lipídico após intervenção
nutricional de 6 semanas desenvolvida com crianças e adolescentes com sobrepeso e
obesidade no Rio de Janeiro.
Sujeitos e métodos: Foi realizado um estudo de intervenção do tipo ensaio clínico
randomizado pragmático com duração de 6 semanas, em um grupo de crianças e
adolescentes (2 a 17 anos de idade) com excesso de peso. Os indivíduos foram
randomizados em um grupo controle (receberam um planejamento alimentar baseado na
estratificação lipídica e redução calórica) e um grupo de intervenção (além desse
planejamento alimentar, receberam 51g de aveia em flocos/dia, totalizando 2,32g de βglucana). O planejamento nutricional foi calculado com base no passo 2 das
recomendações do National Cholesterol Education Program Pediatric Panel (1992).
Realizaram-se análises descritivas das variáveis de estudo, teste t para amostras
independentes para comparações entre os grupos e teste t para amostras pareadas para
comparar o antes e depois em cada grupo.
Resultados: A amostra final constituiu em 110 crianças/adolescentes. Houve redução
estatisticamente significativa no peso, IMC, circunferência de cintura, colesterol,
triglicerídeos e não-HDL-c em ambos os grupos. Porém, o grupo de intervenção
apresentou maiores reduções no colesterol total (-26,8±29,9 mg/dL versus -15,3±24,4
mg/dL; p=0,003), VLDL-c (-10,4±13,8mg/dL versus -2,6±9,9mg/dL; p=0,036) e nãoHDL-c (-24,9±29,9mg/dL versus -10,0±25,4mg/dL; p=0,025).
Conclusão: A adição da aveia em uma dieta hipocalórica e equilibrada em
macronutrientes provocou maiores reduções no colesterol total, VLDL-c e não-HDL-c,
e mostrou-se mais efetiva na diminuição do risco cardiovascular quando comparada a
uma dieta hipocalórica e equilibrada isolada. No entanto, destaca-se a importância da
estratégia nutricional utilizada que se mostrou suficiente para provocar uma diminuição
no peso, IMC e CC e atenuar as alterações no perfil lipídico inclusive daqueles que não
fizeram o uso da aveia.
Abstract
Introduction: The prevalence of childhood obesity has increased in recent decades,
becoming as a global concern. To face it, has been promoted strategies encouraging
healthy eating, besides the interest in functional foods, such as oats.
Objective: To analyze the change in anthropometric and lipid profiles after 6 weeks
dietary intervention conducted with children and adolescents with overweight and
obesity in Rio de Janeiro.
Subjects and methods: A randomized pragmatic clinical trial study for 6 weeks was
conducted, in a group of children and adolescents (2-17 years old) overweight. The
subjects were randomized into a control group (given a meal plan based on lipid
stratification and caloric reduction) and intervention group (beyond this meal plan,
received 51g of oats per day, totaling 2.32 g of β-glucan). Nutritional planning was
calculated in step 2 of the recommendations of the National Cholesterol Education
Program Pediatric Panel (1992). We conducted descriptive analyzes of the study
variables, independent sample t test for comparisons between groups and paired t test to
compare before and after in each group.
Results: The final sample included 110 children/adolescents. There was a statistically
significant reduction in weight, BMI, waist circumference, cholesterol, triglycerides and
non-HDL-c in both groups. However, the intervention group showed greater reductions
in total cholesterol (-26.8 ± 29.9 mg/dL vs. -15.3 ± 24.4 mg/dL, p = 0.003), VLDL-c ( -
10.4 ± 13.8 mg/dL vs. -2.6 ± 9.9 mg/dl, p = 0.036) and non- HDL-c C ( -24.9 ± 29.9
mg/dL vs. -10.0 ± 25.4 mg/dL, p = 0.025).
Conclusion: The addition of oats on a hypocaloric and balanced diet macronutrients
caused greater reductions in total cholesterol, VLDL-c and non- HDL-c, and was more
effective in reducing cardiovascular risk when compared to a balanced hypocaloric diet
alone. However, we highlight the importance of nutritional strategy used which proved
sufficient to cause a decrease in weight, BMI and WC and mitigate the changes in lipid
profile including those who did not use oats.
