O Instituto Fernandes Figueira é um dos poucos hospitais da rede pública com condições técnicas para receber recém-nascidos com malformações congênitas, dentre elas, a mielomeningocele que é um defeito dos arcos vertebrais, e ocorre por falha do fechamento do canal medular. Essas crianças apresentam peculiaridades daquelas acometidas por outras malformações congênitas do sistema nervoso, entre elas, um distúrbio neuromuscular vesical denominado bexiga neurogênica, o qual pode evoluir com dilatação do trato urinário superior e perda da sua função. A urina estagnada constitui um excelente meio para o crescimento de bactérias. O cateterismo vesical intermitente consiste na introdução de uma sonda de PVC na uretra da criança até que se observe a saída de diurese por alguns minutos. O objetivo do trabalho é estudar os fatores que interferem na realização do cateterismo intermitente nos pais de crianças com mielomeningocele. A pesquisa é de natureza qualitativa, e utiliza como instrumento de coleta de dados um questionário de identificação e uma entrevista estruturada. Oito famílias cujas crianças nasceram com mielomeningocele e cujas mães foram treinadas no ambulatório de urodinâmica para a realização da técnica do cateterismo intermitente, foram contactadas através de telefonemas e telegramas, apenas 4 assinaram o termo de consentimento para participar da pesquisa. Muitos fatores foram encontrados no estudo os quais podem interferir no procedimento do cateterismo intermitente. Sendo os mais importantes: malformada, o desconhecimento quanto ao procedimento, falta de suporte financeiro para aquisição do material necessário para a realização do cateterismo, a necessidade de morar com outros membros da família e não poder trabalhar fora. As informações passadas pelos médicos, enfermeiras e auxiliares de enfermagem ajudaram nesse processo de construção do conhecimento sobre o procedimento. Essas informações transmitiram segurança e diminuíram a ansiedade, entretanto, ainda existe a necessidade de um acompanhamento dessa criança e sua família desde a sua internação na UTI até a alta para casa e o acompanhamento ambulatorial.
Abstract
In the state of Rio deJaneiro, Instituto Fernandes Figueira (FIOCRUZ) is one of the few public network hospitals with technical conditions to receive newborns with congenital malformations, among it, the neural tube defect spectrum, which includes the myelomeningocele, group. Myelomeningocele is a vertebral arch defect that occurs because of a failure to close the medullar channel properly. These children have peculiarities similar to those bom_ with other congenital malformations and its complictions, such as visical neuromuscular disturbance denominated-neurogenic-bladder, are frequent. This particular bladder complication can evolveto loss of urinary function because of ascendant dilatation of urinary tract. Thestagnated urine constitutesna excellent media for bacterial growth. Intermittent uretral catheterisation consists in introduction of aclastic probe in the patient's urethra until diuresis is observed. After the patient's hospitalisation outcomehas completed this proceduremust be accomplished and performed by one of the. family members. As partof a neonatal, intensive care unit team, we are responsible for the parents-training regarding- thefteehical assistance for outpatient urethral catheterisation. Psychological uncertainties aspects such as fear and anguish associated toa lack of technical standardisation for the catheterism by the health care professionls increases the high expectations of the family after the hospital leave. The main objective of this work was to study the associated factors among family members of patients with myelomeningocele that could interfere with the intermittent urethral catheterisation, once- leaving the hospital site. The research followed a qualitative methodology comprehending an interview and questionnaire performed at the hospital site. Eight families with children bom
with myelomeningocele and trained by the urodinamic outpatient clinic staff for intermittent urethra catheterism were contacted by phone or letter and only four signed the informed consent for participation in theresearch. Several factors were detected in_our investigation that could interfere in the intermittent urethral catheterisation outcome. Among the main factors we considered as relevant: the severe psychological impact of giving birth to a malformed child, the lack of timing to perform the procedure and the understanding of technical informatics related to the procedure, the lack of financial support by the local government to acquire the instruments for urethral catheterisation, the need of living with parents and relatives and leave the job. The informations passed by the. doctors and nurses and the nursing assesment helped in the process of building knowledge and.confidence to. perform the urethral catheterisatiom Thtconfidence of the parents, in this procedure seems to minimize the psychosocial problems related ter the child and to the family. However, there is a need of following the child and the family sincethestay in the Intensive Care Unit throughtheambulatory careand at home.
