O presente estudo busca ampliar a compreensão do processo de implantação do HÓRUS –
Sistema Nacional de Gestão da Assistência Farmacêutica nos municípios. Este sistema de
informações, desenvolvido em plataforma web e ofertado aos municípios brasileiros pelo
Ministério da Saúde, vem sendo implantado nas Centrais de Abastecimento Farmacêutico
(CAF) e nas Farmácias municipais e estaduais do Sistema Único de Saúde (SUS). Dessa
forma, permite o registro do fluxo de medicamentos, possibilitando o rastreamento e o
controle da distribuição em toda rede de saúde. Espera-se, por meio do entendimento dos
determinantes e dos condicionantes da implantação do HÓRUS, contribuir para aprimorar a
gestão deste sistema. Para tanto, realizou-se um estudo descritivo e exploratório, com
abordagem qualitativa dos dados, a fim de caracterizar o contexto e os aspectos que podem
influenciar na apropriação da tecnologia do HÓRUS pelos municípios e analisar o processo de
implantação nas dimensões técnica-operacional; política-institucional; e organizacional da
Assistência Farmacêutica. A pesquisa contemplou um estudo de caso em um município em
que o HÓRUS vinha sendo utilizado regularmente, aliado a um inquérito eletrônico,
respondido por um conjunto de municípios que iniciaram a implantação do sistema e não
deram continuidade no registro dos dados. No intuito de contribuir para o monitoramento da
implantação do HÓRUS nos municípios são apresentadas algumas considerações a cerca de
elementos facilitadores e fragilidades encontradas na implantação. Estes elementos estão
relacionados ao sistema e a sua gestão nacional, bem como ao local de implantação e a gestão
no município. As fragilidades dizem respeito principalmente à arquitetura online do sistema; à
infraestrutura tecnológica; à rotatividade, insuficiência e resistência dos recursos humanos; a
fragmentação dos sistemas de informação; e ao tempo necessário para que a validade do
sistema seja observada. Como facilitadores, observou-se a natureza pública e a amplitude
nacional do sistema; a organização de rede de suporte; a facilidade na utilização; a discussão
do processo de organização da Assistência Farmacêutica e não apenas da informatização; o
alinhamento com as demais políticas prioritárias do município; a compreensão da importância
das informações; e a realização da implantação mediante um compromisso institucional entre
a gestão e os trabalhadores de saúde. Esta pesquisa reforça que é de suma importância o papel
do gestor no planejamento, na condução da implantação e na institucionalização do uso das
informações nos processos decisórios. Assim como, a incorporação da ferramenta ao dia-a-dia
dos trabalhadores, tanto no que se refere à alimentação dos dados, quanto ao uso das
informações.
Abstract
This study seeks to broaden the understanding of the process of implementation of HORUS -
National System for the Management of Pharmaceutical Care in the municipalities. This
information system, developed web platform and offered to municipalities by the Ministry of
Health, has been deployed in the Pharmaceutical Supply Centers (CAF) and the Unified
Health System (SUS) Pharmacies city and state. Thus, allows recording of the flow of drugs,
enabling the tracking and control of the distribution across the health network. It is hoped,
through the understanding of the determinants and constraints of implementation of HORUS,
contribute to improve the management of this system. Therefore, we performed a descriptive
study and exploratory, qualitative approach in order to characterize the context and the
aspects that can influence in the appropriation of technology of the HORUS by municipalities
and analyze the deployment process in the dimensions technical-operational, political,
institutional, and organizational Pharmaceutical Assistance. The research included a case
study in a county where the HORUS was being used regularly, coupled to an electronic
survey, answered by a group of municipalities that initiated the deployment of the system and
did not continue in the registry data. In order to contribute to the monitoring of the
implementation of HORUS on the municipalities are some considerations about the enablers
and weaknesses found in the deployment. These elements are related to the system and its
national management, as well as the place of deployment and management in the
municipality. The weaknesses relate mainly to the online system architecture, the
technological infrastructure, the turnover rate, insufficiency and resistance of human
resources, the fragmentation of information systems and the time required for the validity of
the system is observed. As facilitators, we observed the public nature and the national scope
of the system, organization support network, the ease of use, the discussion of the process of
organization of pharmaceutical services and not just of the computerization, the alignment
with other priorities policy of the municipality, the understanding of the importance of
information and the realization of implantation via an institutional commitment between
management and workers. This research reinforces that it is extremely important to the
manager's role in planning, conducting implementation and institutionalization of the use of
information in decision making. As well as the incorporation of the tool into the day-to-day
workers, with respect to data input and information use.
