PAPEL DA PROTEÍNA-C REATIVA NO PROGNÓSTICO DO ACIDENTE VASCULAR CEREBRAL ISQUÊMICO

Nos últimos anos pesquisadores vêm tentando uma melhor compreensão do processo aterosclerótico em função de níveis séricos persistentemente aumentados da proteína-C reativa (PC R), cada vez mais envolvida na definição de prognósticos nas lesões do leito arterial coronariano, tanto nos casos agudos quanto nos crônicos. Em função da similaridade entre os processos fisiopatológicos, mais recentemente esses pesquisadores têm voltado sua atenção para uma tentativa de estabelecer essas mesmas relações, antes descritas nas doenças coronarianas, para as doenças cerebro-vasculares, mais propriamente as doenças atero-trombóticas, e sua expressão clínica mais comum, o acidente vascular isquêmico agudo (AVCI), o mais freqüente evento vascular cerebral entre todos os acidentes vasculares cerebrais. Neste trabalho, foram selecionados 27 pacientes com desordens motoras, que foram internados através da Unidade de Emergência do Hospital Português da Bahia (Salvador, Brasil) após tomografia computadorizada, no período de julho a dezembro de 2002, todos com diagnóstico clínico de acidente vascular cerebral isquêmico agudo ou ataque isquêmico transitório (AIT). Após análise do nível sérico de PCR (dosada na admissão e após uma semana da internação por método nefelométrico) e demais variáveis laboratoriais, clínicas e radiológicas, foi verificado que aqueles com quadros de AVCI apresentam valores de PCR mais elevados em comparação com aqueles com AIT, e que estes apresentavam piores desfechos clínicos (alta com seqüelas motoras e morte), a despeito de não ser encontrada diferença estatística significante (p=0.66). Conclusão: a PCR pode fazer parte na avaliação da extensão e da gravidade clínica do AVCI, bem como o prognóstico a curto prazo.
Abstract
In the recent years researchers have been trying to better understand the atherosclerotic process taking into account the persistently high levels of C- reactive protein. C-reactive protein levels have been increasingly used in the definition of prognosis of lesions that take place in the coronary arterial bed, both in acute and chronic diseases. The fact that coronary and cerebrovascular diseases share similar pathological mechanisms has prompted many researchers to also investigate the existence of associations between the former and the levels of C-reactive protein. Most attention has been given to the study of associations involving athero-thrombotic diseases, commonly expressed as acute cerebrovascular ischemic events. In this work, we selected 27 patients with motor disorders that have been admitted in the emergency unity of the Hospital Portugués (a general hospital in Salvador, Brazil) from July to December 2002. All the patients were subject to CT scan of the cranium and had clinical diagnosis of acute ischemic cerebrovascular accident or transient ischemic attack. C-reactive protein was measured in the admission and one week after hospital admission using a nephelometric assay. These data were analyzed together with other clinical and laboratorial variables. Our data show that patients with acute ischemic cerebrovascular accident had higher levels of C-reactive protein than patients with transient ischemic attack. Patients with ischemic cerebrovascular accident also had more unfavorable outcomes (motor sequel and death). Such differences were not statistically different (p=0.66). Conclusion: The CRP it can play an important part in the determination of the extension and clinical gravity of the stroke, as well as in the short term prognosis.
