ASSOCIAÇÃO ENTRE A MUTAÇÃO IL13 GLN144ARG (RS20541) E A INFECÇÃO POR MANSONELLA OZZARDI EM UMA ÁREA ENDÊMICA DA AMAZÔNIA CENTRAL

A mansonelose é uma filariose humana cuja importância para a saúde pública vem sendo crescentemente demonstrada. Na Amazônia, seu principal agente etiológico é Mansonella ozzardi e São Gabriel da Cachoeira (SGC), AM, é uma das principais áreas endêmicas de mansonelose na região. Nessas áreas, alguns indivíduos são aparentemente imunes à infecção e há um grande número de casos assintomáticos de infecção. Uma das hipóteses é que isso esteja ligada ao perfil genético dos indivíduos. A partir do que foi relatado para o tipo raro de oncocercose, chamado sowda, sugere-se que a mutação no gene da citocina IL13 Gln144Arg, SNP rs20541 (A/G), esteja associada à suscetibilidade e ao desenvolvimento clínico da infecção por M. ozzardi em SGC. A fim de avaliar essa associação, foram genotipados 85 indivíduos de SGC para a mutação IL13 Gln144Arg (A/G), sendo 38 indivíduos negativos (MO-) e 47 indivíduos positivos (MO+) para infecção por M. ozzardi. Do total de genótipos identificados, foram 57,32% AA, 29,37% (AG) e 13,41%(GG). Entre MO- (55,26% AA / 34,21% AG / 10,53% GG) e MO+ (59,09% AA / 25% AG / 15,91% GG). Dentre MO+, três foram casos confirmados de co-infecção M. ozzardi-M. perstans, sendo 2 deles com genótipo AA e 1, genótipo GG. Não foi possível observar associação entre IL13 Gln144Arg e a condição de infecção por M. ozzardi ou de co-infecção M. ozzardi-M. perstans. Quando avaliada a carga parasitária dos indivíduos MO+, o genótipo AG vs. AA+GG esteve associado à alta microfilaremia (p=0,039). Não foi possível observar associação de IL13 Gln144Arg e a presença de sintomas entre os indivíduos com alta e baixa microfilaremia. Esses resultados indicam que a população de SGC possui um perfil genético distinto do que tem sido relatado para diferentes populações, havendo uma alta prevalência do alelo mutante IL13 Gln144Arg. Nessa área, indivíduos com e sem a mutação IL13 Gln144Arg são suscetíveis à infecção por M. ozzardi e, inclusive, à co-infecção M. ozzardi-M. perstans, sendo que indivíduos com o genótipo heterozigoto (AG) têm maior pré-disposição a cargas parasitárias mais elevadas. Com isso, confirma-se a hipótese de que a mutação IL13 Gln144Arg tenha implicação sobre o desenvolvimento clínico da mansonelose e não com a suscetibilidade à infecção, assim como visto na oncocercose e sugere-se que o papel da citocina IL13 na imunidade humana se estenda para as filarioses humanas em geral, indo além do que tem sido relatado. Dessa forma, este é o primeiro trabalho a demonstrar a ocorrência da mutação IL13 Gln144Arg no Norte do Brasil, e sua associação com o desenvolvimento clínico da mansonelose em São Gabriel da Cachoeira, AM.
Abstract
Mansonellosis is a human filariasis, whose importance for public health has been increasingly demonstrated. In Amazonia, its major etiological agent is Mansonella ozzardi and São Gabriel da Cachoeira (SGC), AM, is one of the principal mansonellosis endemic areas in the region. In these areas, some individuals are apparently immune to infection and there are a large number of asymptomatic cases of infection. One hypotheses is that this is related to the genetic profile of individuals. Based on what has been reported for the rare type of onchocerciasis, called sowda, it is suggested that the mutation in the cytokine gene IL13 Gln144Arg, SNP rs20541 (A/G), is associated with susceptibility and clinical development of M. ozzardi infection in SGC. In order to evaluate this association, 85 individuals from SGC were genotyped for the IL13 Gln144Arg (A/G) mutation, of which 38 were negative (MO-) and 47 positive (MO+) for M. ozzardi infection. Of the total, the identified genotypes were 57.32% AA, 29.37% AG and 13.41% GG. Between MO-:55.26% AA / 34.21% AG / 10.53% GG and MO+: 59.09% AA / 25% AG / 15.91% GG. Among MO +, there were three confirmed cases of M. ozzardi-M perstans co-infection., being two individuals with genotype AA and one with genotype GG. No association between IL13 Gln144Arg and the M. ozzardi infection or M. ozzardi-M perstans co-infection could be observed. When evaluated the parasite load of MO+ individuals, the genotype AG vs. AA+GG was associated with high microfilaraemia (p=0.039). No association between IL13 Gln144Arg mutation and the presence of symptoms among individuals with high and low microfilaraemia could be observed. These results indicate that the SGC population has a distinct genetic profile than has been reported for different populations, with a high prevalence of the mutant allele IL13 Gln144Arg. In this area, individuals with and without the IL13 Gln144Arg mutation are susceptible to M. ozzardi infection and, even, M. ozzardi-M perstans co-infection, although individuals with the heterozygous genotype have higher predisposition to present elevated parasitic loads. Thus, the hypothesis that the IL13 Gln144Arg mutation has implication on the clinical development of mansonellosis was confirmed, but not on the susceptibility, as seen in onchocerciasis, what suggests that the role of IL13 cytokine in human immunity can be extended to human filariasis in general, going beyond what has been reported. Thus, this is the first work to demonstrate the occurrence of the IL13 Gln144Arg mutation in the North of Brazil, and its association with the clinical development of mansonellosis in São Gabriel da Cachoeira, AM.
