A pandemia de aids é bastante complexa e é configurada como um verdadeiro mosaico
de sub-epidemias, apresentando importantes mudanças na sua dinâmica de expansão e
no perfil epidemiológico, desde o aparecimento dos primeiros casos, há
aproximadamente duas décadas, em diferentes regiões do mundo. A cidade do Recife,
com 1.421.993 habitantes, registrou os primeiros casos de aids em meados dos anos 80
e detém uma das maiores taxas de incidência da doença na região nordeste. Na hipótese
de que o avanço da epidemia estaria acompanhado por uma mudança do perfil
epidemiológico dos doentes, este estudo analisa a evolução do perfil da população
atingida para os períodos de 1985-1988, 1989-1992, 1993-1996 e 1997-2000, segundo
características de sexo, escolaridade, categoria de exposição, idade e bairro de
residência. Para a identificação de fatores associados às mudanças no perfil da
epidemia, procedeu-se a análise de variância (ANOVA) das variáveis selecionadas em
relação aos períodos de diagnósticos e elaborou-se mapas temáticos para a análise de
tendência espaço-temporal. Os dados usados para construção de taxas e indicadores
sociodemográficos foram os casos de aids notificados, e para a construção de mapas
temáticos, o mapa da cidade segundo bairro. Dos 2.243 casos estudados, 1802 (80,3%)
eram homens e 441 mulheres, apesar de ter-se verificado um aumento progressivo da
participação feminina, representado pela diminuição da razão de sexo (M:F) que passa
de 21:1, nos primeiros anos da epidemia, para 2,7:1 no período mais recente. A
incidência média global foi de 10,68 por 100.000 habitantes, com um aumento de 2
casos por 100.000 hab. no período inicial (1985-1998) para 16 casos por 100.000 hab.
no período mais recente (1997-2000). Os mapas mostraram uma expansão dos casos de
aids a partir da área central (portuária) e sul (litorânea) para as regiões mais periféricas
da cidade. Dos 94 bairros do Recife, 88 (93,6%) já registraram pelo menos um caso de
aids, os bairros com as maiores taxas de incidência foram o do Recife (197 casos por
100 mil habitantes), Santo Antônio (43,70 casos por 100 mil habitantes) e Cidade
Universitária (45,70 casos por 100 mil habitantes). Apenas os bairros da Jaqueira,
Monteiro, Pau Ferro, Torreão, Sancho e Paissandu, não registraram nenhum caso de
aids, até 2000. À análise das variáveis selecionadas em relação aos períodos de
diagnóstico observou-se uma redução significativa da proporção de casos de média e
alta escolaridade, ao longo do tempo (F=28,6; gl=3;1767; p=0,000), em relação aos de
baixa escolaridade e analfabetos. Também foi verificada uma redução estatisticamente
significante da proporção de casos não-heterossexuais (grupo de casos de homens que
fazem sexo com homens, e todas as categorias de transmissão sangüínea) em relação
aos casos cuja categoria de exposição fora o contato heterossexual (F=44,5; gl=3;1802;
p=0,000) e, da mesma forma, uma redução significativa da participação dos casos do
sexo masculino em todos os períodos da epidemia (F=23,8; gl= 3;2239; p=0,000). Por
outro lado, embora não tenham sido significantes as diferenças de proporção dos casos
com menos de 35 anos quando comparados aos de idade superior ou igual a 35 anos, ao
longo da epidemia (F=1,8; gl=3;2237; p=0,139), o teste de Duncan identifica que o
período inicial tem heterogeneidade significativa em relação aos demais períodos
(p<5%), ou seja, a proporção de casos mais jovens é significativamente maior nos
primeiros anos da epidemia. A crescente expansão da aids no Recife, marcada pelos
processos da heterossexualização, da feminização, da periferização e da pauperização da
epidemia, aponta para a necessidade de adoção de uma política de intervenção pautada
em ações focalizadas, prioritariamente, em grupos populacionais de maior
vulnerabilidade social e de baixos estratos socioeconômicos.
Abstract
The Aids epidemic is quite complex and is made up of a real mosaic of sub-epidemics,
presenting important changes in its dynamic of expansion and in its epidemiological
profile, beginning with the appearance of the first cases, approximately two decades
ago, in different regions of the world. The city of Recife, with 1,421,993 inhabitants,
registered its first cases of Aids in the mid-80 and has one of the highest number of
cases of the disease in the Northeastern Region. On the hypothesis that the advance of
the epidemic is accompanied by a change in the epidemiological profile of the patients,
this study analysis the evolution of the profile of the people affected in the periods
1985-1988, 1989-1992, 1993-1996 and 1997-2000, according to characteristics of
gender, schooling, category of exposure, age and neighborhood. In identifying factors
associated with the changes in the profile of the epidemic, we applied a variance
analysis (ANOVA) of the variables selected in relation to the periods of diagnoses; and
thematic maps were elaborated to analyze the time-space tendency. The data used in the
construction of the sociodemographic rates and indicators was that from the reported
cases of Aids; and for the construction of the thematic maps, the map of the city
according to neighborhood. Of the 2,243 cases studied, 1,802 (80.3%) were men and
441 women, though it was verified that there has been a progressive increase in female
participation, represented by the decrease in the ratio figure (M:F), which went from
21:1, in the first years of the epidemic to 2.7:1 in the most recent period. The average
global figure was 10.68 for each 100,000 inhabitants, with an increase of 2 cases per
100,000 inhabitants in the initial period (1985-1998) to 16 cases per 100,000 cases in
the most recent period (1997-2000). The maps have shown an expansion of the cases of
Aids beginning in the central (the dock ) and south (coastland) areas and spreading to
the more suburban areas of the city. Of the 94 neighborhoods in Recife, 88 (93.6%),
have registered at least one case of Aids, the neighborhoods that have registered the
greatest indexes of occurrence are Recife Antigo (197 cases per 100 thousand
inhabitants), Santo Antônio (43.70 cases per 100 thousand inhabitants) and Cidade
Universitária (45.70 cases per 100 thousand inhabitants). Only the neighborhoods of
Jaqueira, Monteiro, Pau Ferro, Torreão, Sancho and Paissandu, have not registered a
single case of Aids, up to the year 2000. In an analysis of the variables selected in
relation to the periods of diagnosis, it was observed that there has been a significant
reduction in the proportion of cases among those of middle and upper schooling, in the
course of time (F=28.6; gl = 3;1767; p=0.000), in relation to those of low schooling and
illiterates. It was also verified that there has been a significant statistical reduction in the
cases of non-heterosexuals (a group of cases of men who have sex with men, and all the
categories of blood transmission) in relation to the cases in which the category of
exposure was heterosexual contact (F=44.5; gl=3;1802; p=0.00) and, in the same
manner, a significant reduction in the participation of cases of the male sex in all the
periods of the epidemic (F=23.8; gl=3;2239; p=0.000). On the other hand, though the
differences in the proportion of cases of those under 35 years of age, were not
significant, when compared to those over 35 or 35, in the course of the epidemic (F=1.8;
gl=3;2237; p=0.139), the Duncan test shows that the initial period has significant
heterogeneity in regard to all the other periods (p<5%), in other words, the proportion of
cases among the younger is significantly greater in the first years of the epidemic. The
expanding growth of Aids in Recife, marked by the processes of heterosexualization,
feminization, suburbanization and pauperization of the epidemic, points to the need for
the adoption of a policy of intervention regulated by works that give precedence to
populous groups of greater social vulnerability and from the lower socioeconomic
stratum.
