A interrupção prematura do tratamento da tuberculose constitui um dos motivos do insucesso no controle da doença. Buscando reduzir as taxas de abandono e o controle da doença no Brasil, o Programa Nacional de Controle da Tuberculose (PCT) instituiu o tratamento supervisionado (DOTS), em 1999, e para garantir a operacionalização da estratégia, recomendou a descentralização das ações de controle a todos os serviços de saúde. Obedecendo a essa diretriz, a Secretaria de Saúde do Recife iniciou o processo de descentralização do PCT, a partir de 2001, porém o impacto sobre as taxas de abandono ainda não foi analisado. O Objetivo do trabalho foi descrever a evolução das taxas de abandono de tratamento de tuberculose e analisar a sua relação com o processo de descentralização das ações do Programa de Controle da Tuberculose no município do Recife, no período de 1996 a 2005. O estudo foi do tipo ecológico, de série histórica. Utilizou-se como fonte de dados o banco do SINAN. Analisou-se a tendência temporal das taxas de abandono do tratamento e dos percentuais de unidades com PCT implantado no município e por Distrito Sanitário (DS). A associação entre as variáveis foi testada utilizando-se o coeficiente de correlação de Pearson e o modelo de regressão linear. Estimou-se o coeficiente angular, o coeficiente de determinação (R2) e o p-valor a um nível de significância de 5 por cento. Observou-se uma tendência de redução estatisticamente significante das taxas de abandono ao tratamento de TB, no município como um todo e nos DS I, III, IV e VI ao longo da série temporal, mas esta taxa permaneceu, no período, bem acima da meta preconizada pelo Ministério da Saúde. Nos DSs II e V esta redução não foi significativa. Em todos os DSs e no Recife, houve uma significante tendência de aumento de descentralização do programa ao longo da série histórica, e este aumento foi significativamente associado com uma leve redução da taxa de abandono no Recife e no DS IV, nos outros DSs tal associação não se mostrou significativa. Mesmo com o intenso processo de descentralização do PCT para as unidades básicas de saúde, as taxas de abandono ao tratamento da TB, no Recife, continuaram elevadas. Tal situação demonstra que este processo, não se mostrou eficaz para a redução do percentual de abandono no município, durante o período em estudo
Abstract
The premature interruption of tuberculosis treatment is one of the reasons of failure
in controlling the disease. Seeking reduce abandonment rates and control of disease
in Brazil, the National Program for Control of Tuberculosis (PCT) established the
supervised treatment (DOTS), in 1999, and to ensure the operationalization of the
strategy, recommended the decentralization of the shares to control the all health
services. Obeying this directive, the Secretary of Health of Recife started the process
of decentralization of the PCT, from 2001, but the impact on rates of abandonment
has not yet been analyzed. To describe the evolution of the rates of tuberculosis
treatment discontinuation and analyze its relationship with the process of
decentralization of shares of Tuberculosis Control Program in the city of Recife, in
the period from 1996 to 2005. The study was kind of ecological, historical series. It
was used as a source of the data bank of SINAN. Analyzed to time trend in rates of
treatment abandonment and percentage of units with PCT deployed in the city and
by Health District (DS). The association between variables was tested using the
Pearson correlation coefficient and linear regression model. It was estimated the
slope, the coefficient of determination (R2) and the p-value for a significance level of
5%. There was a tendency for a statistically significant reduction in the rates of
abandonment to the treatment of TB, in the municipality as a whole and the DS I, III,
IV and VI during the time series, but the rate remained in the period, and above the
target recommended by the Ministry of Health In DSs II and V this reduction was not
significant. In all DSs in Recife, there was a significant trend of increasing
decentralization of the program along the historical series, and this increase was
significantly associated with a slight reduction in the rate of abandonment in Recife
and the DS IV, in other DSs such association was not significant. Even with the
intense process of decentralization of the PCT for primary health, abandonment rates
to the tuberculosis treatment, in Recife, remained high, shows this process, was not
a measure for reducing the effective percentage of abandonment in the municipality
during the period under study.
