Em Ji-Paraná a intensidade de transmissão da Leishmaniose Tegumentar Americana
(LTA) está associada às atividades laborais, nas quais os indivíduos deslocam para
frentes de trabalho situadas em outros municípios de Rondônia ou para outros estados
vizinhos, cujas condições de receptividade são favoráveis à transmissão da doença. Esse
trabalho teve como objetivo analisar as principais características do processo endêmicoepidêmico da LTA no Município de Ji-Paraná, no período de 2002 a 2008, considerando
as transformações socioambientais ocorridas em função dos processos produtivos e de
trabalho instalados na região. Foi realizado um estudo descritivo através de dados
secundários. Foram georreferenciados os endereços de residência dos doentes e
construídos mapas de fluxo dos casos de LTA. Foi realizada uma estratificação
considerando os locais de infecção dos pacientes. No período estudado foram
notificados 458 casos novos de LTA, onde somente 8,1% acometeram mulheres e
99,1%, atingiram pessoas acima de 10 anos de idade. A forma cutânea da doença
prevaleceu (87,8 %). A distribuição dos casos por local de infecção mostrou que 16,4%
se infectaram no próprio município, sendo a área rural responsável pela totalidade
desses registros. Outros 38 municípios de Rondônia foram citados como provável fonte
de infecção, correspondendo a 33% dos casos. Já 41,9% das notificações foi no estado
do Mato Grosso, resultado das atividades laborais ligadas diretamente à floresta,
evidenciando mudanças na dinâmica de transmissão da doença, que antes se dava em
decorrência da ocupação do solo no próprio município. Foram selecionados três
principais municípios do Mato Grosso, citados como local de infecção da LTA dos
residentes em Ji-Paraná, para estudo dos padrões de transmissão, Aripuanã,
Rondolândia e Colniza, por terem contribuído com 87,6% dos casos.
Este estudo sugere que o processo de ocupação e organização do espaço em Ji-Paraná,
nos últimos 40 anos, propiciou o deslocamento dos indivíduos para locais onde a
transmissão da LTA é mais intensa. O desconhecimento sobre a doença e os cuidados
necessários para evitá-la, contribuem para manutenção dos casos. Ações de educação
em saúde e a descentralização do diagnóstico e tratamento da LTA são indispensáveis
para a redução da doença no município de Ji-Paraná.
Abstract
In Ji-Paraná the intensity of transmission of Leishmaniose American Tegumentar (LAT)
it is associated with the labor activities, in which the individuals dislocate to the front of
work in other cities of Rondônia or for other neighboring states, whose conditions of
receptivity are favorable to the transmission of the illness. This work has an objective to
analyze the main characteristics of the endemic-epidemic process of the LAT in the City
of Ji-Paraná, on the period of 2002 to 2008, considering occurred the social and
environmental transformations in function of the productive processes and work
installed in the region. A descriptive study through secondary data was carried through.
The addresses of residence of the sick people and constructed maps of flow of the LAT
cases had been georeferenced. A stratification was carried through considering the
places of infection of the patients. In the studied period 458 new cases of LAT had been
notified, where 8.1% had only attacked women and 99.1%, they had reached people
above of 10 years old. The cutaneous form of the illness prevailed (87.8%). The
distribution of the cases for infection place showed that 16.4% if infected in the proper
city, being the responsible rural area for the totality of these registers. Others 38 cities of
Rondônia had been cited as probable source of infection, corresponding 33% of the
cases. Already 41.9% of the notifications were in the state of the Mato Grosso, results
of the labor activities related to the forest, evidencing changes in the dynamics of
transmission of the illness, that earlier was given in result of the occupation of the
ground in to proper City. Three main cities of Mato Grosso, cited as local of infection of
the LAT of the residents in Ji-Paraná had been selected, for study of the standards of
transmission, Aripuanã, Rondolândia and Colniza, for having contributed with 87,6% of
the cases. This study suggests that the process of occupation and organization of the
space in Ji-Paraná, the last 40 years, propitiated the displacement of the individuals for
places where the transmission of the LAT is more intense. The unfamiliarity on the
illness and the cares necessary to prevent it, contributes for the maintenance of the
cases. Actions of education in health and the decentralization of the diagnosis and
treatment of the LAT are indispensable for the reduction of the illness in the city of JiParaná.
