A construção efetiva de políticas públicas de gestão e de trabalho na Atenção Primária à
Saúde envolve uma alta complexidade, que não se traduz pelo uso de tecnologias
avançadas de alto custo, mas sim com as interferências do território nos processos de
cuidado. Essas interferências possuem uma dimensão não só técnica como também
ético-política. O território possui redes existenciais, políticas, locais, formais e/ou
informais. O processo trabalho em saúde, baseado no território e suas redes, requer uma
atenção inclusiva ao que nele, através dele e por ele surge através de seus movimentos.
O território e seus determinantes sociais envolvem, produzem e tensionam os processos
de gestão, produção e promoção da saúde, criando uma complexidade nas práticas da
Atenção Primária à Saúde, justificando práticas que se voltem para o enfrentamento dos
desafios e tensionamentos cotidianos que não são impedimentos, mas devem ser
considerados matéria viva que promoverá o trabalho em saúde na Atenção Primária.
Com esse balizamento o objetivo desta pesquisa é articular uma discussão teórica a um
estudo de caso, que desvele as múltiplas dimensões do território, seus dispositivos e sua
influência na estruturação de uma prática de gestão na Estratégia Saúde da Família
comprometida com os problemas e as necessidades da população e com as demandas
dos serviços. O campo da pesquisa foi uma Clínica da Família da Área Programática 3.2
do município do Rio de Janeiro, onde foram colhidas as falas dos sujeitos da pesquisa
através de entrevistas semi-estruturadas e posteriormente estas falas foram analisadas e
sistematizadas através do Discurso do Sujeito Coletivo, obtendo assim as
Representações Sociais do grupo investigado, a compreensão das concepções de
território que os profissionais da Estratégia saúde da Família e a comunidade têm e o
reconhecimento da importância de se apropriar dos dispositivos do território para
melhorar a gestão do cuidado.
Abstract
The effective construction of public management policies and working in Primary
Health Care involves a high complexity, which is not translated by the use of advanced
high cost, but with the interference of the territory in the processes of care. These
interferences have a dimension not only technical but also ethical and political. The
territory has existential political, local, formal and / or informal networks. The work
process in health care, based on territory and their networks, requires inclusive attention
itself, through it and it comes from its own movements. The territory and its social
determinants involve, produce and creates tension on management processes,
production and promotion of health, creating complexity in the practice of Primary
Health Care, justifying practices that turn to face the everyday challenges and tensions
that are not impediments but should be considered living matter that promote health
work in Primary Care. With these considerations, the goal of this research is to
articulate a theoretical discussion to a case study that unveils the multiple dimensions of
the territory, their devices and their influence on the structuring of a management
practice in a Family Health Strategy committed with population’s problems and needs
and service’s demand. The research field was one of the Family Clinics of the 3.2
Program Area of Rio de Janeiro municipality, where the speeches lines were taken of
the individuals through semi-structured interviews and those lines were subsequently
analyzed and systematized through the Discourse of the Collective Subject, thus
obtaining the social representations of the investigated group, understanding the
concepts of territory that health professionals from Family Health Strategy and
community have, and also the recognition of the importance of appropriating territory’s
devices to improve care management.
