O trabalho discute as principais barreiras enfrentadas pelos adolescentes na adesão à
terapia antirretroviral em um contexto específico: o Serviço de Doenças Infecciosas do
Hospital dos Servidores do Estado, na cidade do Rio de Janeiro. A faixa etária
compreendida neste estudo foi de 10 a 24 anos seguindo critérios da Organização
Mundial da Saúde e do Programa de Saúde do Adolescente do Ministério da Saúde. Os
pacientes foram previamente selecionados a partir da análise de seus dados disponíveis
nos prontuários e de entrevistas semi-estruturadas realizadas com a médica e a
enfermeira responsáveis pelo ambulatório. Entre os adolescentes selecionados, foram
entrevistados oito pacientes, seis do sexo feminino e dois do sexo masculino, de 18 a 24
anos. A partir do material obtido nas entrevistas foram definidas as seguintes dimensões
de análise: revelação/aceitação; viver com HIV e AIDS; suporte da equipe de saúde e
da família e cumprimento terapêutico, e sua relação ao processo de adesão aos
antirretrovirais. Os adolescentes enfatizaram as principais dificuldades enfrentadas no
dia a dia: preconceito, discriminação e estigma, que podem causar sentimentos que
tendem a deixá-los vulneráveis ao abandono do tratamento. A aceitação da doença é um
processo constante, sempre momentâneo e pontual. O apoio dos familiares e da equipe
de saúde, através do “grupo de jovens” foram primordiais no processo de adesão ao
tratamento. Todos os adolescentes, embora reconheçam a importância de ingerir seus
medicamentos corretamente, relataram dificuldades para seguir o tratamento à risca. O
futuro é visto com insegurança por todos os entrevistados, mas mesmo assim, sonhos
como constituir uma família e criar os filhos tem vez na vida destes jovens. Os
resultados deste estudo demonstram que a adesão vai muito além de tomar o
medicamento certo, na hora certa e na dose certa e que não é possível predizer os
motivos que levam à não adesão, pois estes são fatos peculiares e próprios da história de
cada indivíduo.
Abstract
The present work discusses the main obstacles faced by adolescents to adhere the
antiretroviral therapy in a specific context: the Infectious Disease Division of the State
Servants Hospital (Hospital dos Servidores do Estado) in the city of Rio de Janeiro.
The age of the participants ranged from 10 to 24 years in accordance with the World
Health Organization and the Adolescent Health Program of the Ministry of Health. The
patients were selected on the basis of the data available in their medical charts and the
semi-structured interviews with the doctor and the nurse in charge of the clinic. Among
the selected adolescents, eight patients (ranging from 18 to 24 years of age) were
interviewed, six females and two males. From the result of the interviews, some
analysis criteria were defined: revelation/acceptance; living with HIV and AIDS; health
staff and family support and therapy compliance, in relation to the adherence to the
antiretroviral therapy. The adolescents highlighted the main difficulties they had on
their daily routines: prejudice, discrimination and stigma, which make them vulnerable
to the point of abandoning treatment. The acceptance of the disease is a constant
process, always temporary and punctual . Health staff and family support, through the
“youth group”, were critical for the adherence to the treatment. All adolescents, despite
being aware of the importance of taking the drugs correctly, reported difficulties in
complying with the treatment. Future is seen with uncertainty by all interviewees,
however, these youngsters still have dreams of having a family and raising children.
The results of this study show that the adherence to the treatment not only depends on
taking the correct doses of drugs at the right time, but also on subjective and particular
factors that are difficult to predict.
