A infecção pelo HIV representa um grave problema de Saúde Pública e tem atingido de forma especial as mulheres, aumentando o risco de transmissão vertical (TV). O surgimento de antirretrovirais mais potentes para o controle da TV trouxe forte impacto na sobrevida de crianças com essa infecção. Entretanto esses medicamentos podem levar ao baixo peso e à prematuridade nos bebês, comprometendo as condições de saúde dos recém-nascidos.Objetivo. Determinar o peso ao nascer e mortalidade infantil, segundo o status de HIV da mãe, tratamento, e fatores sociodemográficos e clinico-obstétricos. Métodos. Foi realizado um estudo observacional de coorte retrospectiva de nascidos do município do Rio de Janeiro no período de 2007 a 2013, através das bases do SINAN, SINASCe SIM e para efeito de comparação entre a coorte de filhos de mães HIV+ e o conjunto dos filhos das mães HIV-nascidos do mesmo período foi utilizada a técnica de linkage. Para análise de dados foram utilizados os testes t-student, teste-U de Mann-Whitneye o teste de Kruskal-Wallis. As diferenças entre as proporções das variáveis categóricas foram avaliadas pelo teste qui quadrado de Pearson. Em todas as análises foi considerado um grau de significância de 5%. As magnitudes das associações entre statusdeinfecção materna ao HIV e o baixo peso ao nascer foram avaliadas pelo cálculo das Razões de Chances (RC) brutas e ajustadas, estimadas através da estatística de Regressão Logística Múltipla, com seus respectivos intervalos de confiança 95%. As estimativasdo risco de óbito em 1 ano, segundo o status de HIV materno e status de BPN foram estimadas através do cálculo da mortalidade infantil. Todas as análises serão feitas utilizando o pacote estatístico SPSS (versão 20.0). Resultados. Filhos de mães HIV+ apresentaram uma chance de nascerem com BP 2,38 vezes maior, independentemente das demais variáveis. Crianças nascidas de HIV + que fizeram TARV no pré-natal apresentaram uma chance de ter BP ao nascer 2,57 vezes maior, enquanto filhos de mães que sem TARV esta chance foi 1,24 vezes maior. O risco de óbito em 1 ano foi estatisticamente maior entre os filhos de mães HIV+ que nasceram com peso < 1000g (20%),(log-Rank test, p-valor = 0,000). Conclusão. A infecção materna ao HIV aumentou significativamente o riscode BPN na coorte de estudo, quando comparadas às gestantes HIV-. Dentre os nascidos vivos de mães HIV+, o tratamento com TARV no pré-natal parece ter afetado negativamente o peso ao nascer.
Abstract
HIV infection is a serious public health problem and has hit women in particular, increasing the risk of vertical transmission (TV). The emergence of more potent antiretrovirals for the control of the TV hada strong impact on the survival of children with this infection. However, these medications can lead to low birth weight and prematurity, compromising the health of newborns. Goal. To determine birth weight and infant mortality according to the mother's HIV status, treatment, and socio-demographic and clinical-obstetric factors. Methods. An observational retrospective cohort study was carried out on the births of the city of Rio de Janeiro from 2007 to 2013, using the bases of SINAN, SINASC and SIM and forthe comparison between the cohort of children of HIV + mothers and the set of children Of the HIV-born mothers of the same period was used the linkage technique. For data analysis, the t-student, Mann-Whitney U-test and the Kruskal-Wallis test were used. The differences between the proportions of the categorical variables were assessed by the Pearson chi-square test. A significance level of 5% was considered in all analyzes. The magnitudes of the associations between maternal HIV infection status and low birth weight were assessed by calculating the Gross and Adjusted Chances Ratios (RC), estimated through the Multiple Logistic Regression Statistics, with their respective 95% confidence intervals. Estimates of death risk at 1 year, according to maternal HIVstatus and LBW status were estimated by calculating infant mortality. All analyzes will be done using the SPSS statistical package (version 20.0). Results. Sons of HIV + mothers presented a chance of being born with BP 2.38 times higher, independently of the other variables. Children born to HIV + who had ART in prenatal care had a chance of having BP at birth 2.57 times higher, whereas children of mothers without ART had a chance of 1.24 times higher. The risk of death at 1 year was statistically higher among the children of HIV + mothers who were born weighing <1000g (20%), (log-rank test, p-value = 0.000). Conclusion. Maternal HIV infection significantly increased the risk of LBW in the study cohort when compared to HIV-pregnant women. Among the live births of HIV + mothers, treatment with ART in prenatal care seems to have negatively affected birth weight.
